8a244590ca x86_64: introduce CalIOC2 support
CalIOC2 is a PCI-e implementation of the Calgary logic. Most of the
programming details are the same, but some differ, e.g., TCE cache
flush. This patch introduces CalIOC2 support - detection and various
support routines. It's not expected to work yet (but will with
follow-on patches).

ef3e28c5b9 x86_64: check remote IRR bit before migrating level triggered irq
On x86_64 kernel, level triggered irq migration gets initiated in the
context of that interrupt(after executing the irq handler) and following
steps are followed to do the irq migration.

1. mask IOAPIC RTE entry;     // write to IOAPIC RTE
2. EOI;                       // processor EOI write
3. reprogram IOAPIC RTE entry // write to IOAPIC RTE with new destination and
                              // and interrupt vector due to per cpu vector
                              // allocation.
4. unmask IOAPIC RTE entry;   // write to IOAPIC RTE

Because of the per cpu vector allocation in x86_64 kernels, when the irq
migrates to a different cpu, new vector(corresponding to the new cpu) will
get allocated.

An EOI write to local APIC has a side effect of generating an EOI write for
level trigger interrupts (normally this is a broadcast to all IOAPICs).
The EOI broadcast generated as a side effect of EOI write to processor may
be delayed while the other IOAPIC writes (step 3 and 4) can go through.

Normally, the EOI generated by local APIC for level trigger interrupt
contains vector number.  The IOAPIC will take this vector number and search
the IOAPIC RTE entries for an entry with matching vector number and clear
the remote IRR bit (indicate EOI).  However, if the vector number is
changed (as in step 3) the IOAPIC will not find the RTE entry when the EOI
is received later.  This will cause the remote IRR to get stuck causing the
interrupt hang (no more interrupt from this RTE).

Current x86_64 kernel assumes that remote IRR bit is cleared by the time
IOAPIC RTE is reprogrammed.  Fix this assumption by checking for remote IRR
bit and if it still set, delay the irq migration to the next interrupt
arrival event(hopefully, next time remote IRR bit will get cleared before
the IOAPIC RTE is reprogrammed).

bd78432c8f x86_64: mcelog tolerant level cleanup
Background:
 The MCE handler has several paths that it can take, depending on various
 conditions of the MCE status and the value of the 'tolerant' knob.  The
 exact semantics are not well defined and the code is a bit twisty.

Description:
 This patch makes the MCE handler's behavior more clear by documenting the
 behavior for various 'tolerant' levels.  It also fixes or enhances
 several small things in the handler.  Specifically:
     * If RIPV is set it is not safe to restart, so set the 'no way out'
       flag rather than the 'kill it' flag.
     * Don't panic() on correctable MCEs.
     * If the _OVER bit is set *and* the _UC bit is set (meaning possibly
       dropped uncorrected errors), set the 'no way out' flag.
     * Use EIPV for testing whether an app can be killed (SIGBUS) rather
       than RIPV.  According to docs, EIPV indicates that the error is
       related to the IP, while RIPV simply means the IP is valid to
       restart from.
     * Don't clear the MCi_STATUS registers until after the panic() path.
       This leaves the status bits set after the panic() so clever BIOSes
       can find them (and dumb BIOSes can do nothing).

 This patch also calls nonseekable_open() in mce_open (as suggested by akpm).

Result:
 Tolerant levels behave almost identically to how they always have, but
 not it's well defined.  There's a slightly higher chance of panic()ing
 when multiple errors happen (a good thing, IMHO).  If you take an MBE and
 panic(), the error status bits are not cleared.

Alternatives:
 None.

Testing:
 I used software to inject correctable and uncorrectable errors.  With
 tolerant = 3, the system usually survives.  With tolerant = 2, the system
 usually panic()s (PCC) but not always.  With tolerant = 1, the system
 always panic()s.  When the system panic()s, the BIOS is able to detect
 that the cause of death was an MC4.  I was not able to reproduce the
 case of a non-PCC error in userspace, with EIPV, with (tolerant < 3).
 That will be rare at best.

e02e68d31e x86_64: support poll() on /dev/mcelog
Background:
 /dev/mcelog is typically polled manually.  This is less than optimal for
 situations where accurate accounting of MCEs is important.  Calling
 poll() on /dev/mcelog does not work.

Description:
 This patch adds support for poll() to /dev/mcelog.  This results in
 immediate wakeup of user apps whenever the poller finds MCEs.  Because
 the exception handler can not take any locks, it can not call the wakeup
 itself.  Instead, it uses a thread_info flag (TIF_MCE_NOTIFY) which is
 caught at the next return from interrupt or exit from idle, calling the
 mce_user_notify() routine.  This patch also disables the "fake panic"
 path of the mce_panic(), because it results in printk()s in the exception
 handler and crashy systems.

 This patch also does some small cleanup for essentially unused variables,
 and moves the user notification into the body of the poller, so it is
 only called once per poll, rather than once per CPU.

Result:
 Applications can now poll() on /dev/mcelog.  When an error is logged
 (whether through the poller or through an exception) the applications are
 woken up promptly.  This should not affect any previous behaviors.  If no
 MCEs are being logged, there is no overhead.

Alternatives:
 I considered simply supporting poll() through the poller and not using
 TIF_MCE_NOTIFY at all.  However, the time between an uncorrectable error
 happening and the user application being notified is *the*most* critical
 window for us.  Many uncorrectable errors can be logged to the network if
 given a chance.

 I also considered doing the MCE poll directly from the idle notifier, but
 decided that was overkill.

Testing:
 I used an error-injecting DIMM to create lots of correctable DRAM errors
 and verified that my user app is woken up in sync with the polling interval.
 I also used the northbridge to inject uncorrectable ECC errors, and
 verified (printk() to the rescue) that the notify routine is called and the
 user app does wake up.  I built with PREEMPT on and off, and verified
 that my machine survives MCEs.

f528e7ba28 x86_64: O_EXCL on /dev/mcelog
Background:
 /dev/mcelog is a clear-on-read interface.  It is currently possible for
 multiple users to open and read() the device.  Users are protected from
 each other during any one read, but not across reads.

Description:
 This patch adds support for O_EXCL to /dev/mcelog.  If a user opens the
 device with O_EXCL, no other user may open the device (EBUSY).  Likewise,
 any user that tries to open the device with O_EXCL while another user has
 the device will fail (EBUSY).

Result:
 Applications can get exclusive access to /dev/mcelog.  Applications that
 do not care will be unchanged.

Alternatives:
 A simpler choice would be to only allow one open() at all, regardless of
 O_EXCL.

Testing:
 I wrote an application that opens /dev/mcelog with O_EXCL and observed
 that any other app that tried to open /dev/mcelog would fail until the
 exclusive app had closed the device.

Caveats:
 None.

When we are in the emulated NUMA case, we need to make sure that all existing
apicid_to_node mappings that point to real node ID's now point to the
equivalent fake node ID's.

If we simply iterate over all apicid_to_node[] members for each node, we risk
remapping an entry if it shares a node ID with a real node.  Since apicid's
may not be consecutive, we're forced to create an automatic array of
apicid_to_node mappings and then copy it over once we have finished remapping
fake to real nodes.

3484d79813 x86_64: fake pxm-to-node mapping for fake numa
For NUMA emulation, our SLIT should represent the true NUMA topology of the
system but our proximity domain to node ID mapping needs to reflect the
emulated state.

When NUMA emulation has successfully setup fake nodes on the system, a new
function, acpi_fake_nodes() is called.  This function determines the proximity
domain (_PXM) for each true node found on the system.  It then finds which
emulated nodes have been allocated on this true node as determined by its
starting address.  The node ID to PXM mapping is changed so that each fake
node ID points to the PXM of the true node that it is located on.

If the machine failed to register a SLIT, then we assume there is no special
requirement for emulated node affinity so we use the default LOCAL_DISTANCE,
which is newly exported to this code, as our measurement if the emulated nodes
appear in the same PXM.  Otherwise, we use REMOTE_DISTANCE.

PXM_INVAL and NID_INVAL are also exported to the ACPI header file so that we
can compare node_to_pxm() results in generic code (in this case, the SRAT
code).

3af044e0f8 x86_64: extract helper function from e820_register_active_regions
The logic in e820_find_active_regions() for determining the true active
regions for an e820 entry given a range of PFN's is needed for
e820_hole_size() as well.

e820_hole_size() is called from the NUMA emulation code to determine the
reserved area within an address range on a per-node basis.  Its logic should
duplicate that of finding active regions in an e820 entry because these are
the only true ranges we may register anyway.

34feb2c83b x86_64: Quicklist support for x86_64
This adds caching of pgds and puds, pmds, pte.  That way we can avoid costly
zeroing and initialization of special mappings in the pgd.

A second quicklist is useful to separate out PGD handling.  We can carry the
initialized pgds over to the next process needing them.

Also clean up the pgd_list handling to use regular list macros.  There is no
need anymore to avoid the lru field.

Move the add/removal of the pgds to the pgdlist into the constructor /
destructor.  That way the implementation is congruent with i386.

8180a55028 x86_64: hpet tsc calibration fix broken smi detection logic
The current SMI detection logic in read_hpet_tsc() makes sure,
that when a SMI happens between the read of the HPET counter and
the read of the TSC, this wrong value is used for TSC calibration.

This is not the intention of the function. The comparison must ensure,
that we do _NOT_ use such a value.

Fix the check to use calibration values where delta of the two TSC reads
is smaller than a reasonable threshold.

2aae950b21 x86_64: Add vDSO for x86-64 with gettimeofday/clock_gettime/getcpu
This implements new vDSO for x86-64.  The concept is similar
to the existing vDSOs on i386 and PPC.  x86-64 has had static
vsyscalls before,  but these are not flexible enough anymore.

A vDSO is a ELF shared library supplied by the kernel that is mapped into
user address space.  The vDSO mapping is randomized for each process
for security reasons.

Doing this was needed for clock_gettime, because clock_gettime
always needs a syscall fallback and having one at a fixed
address would have made buffer overflow exploits too easy to write.

The vdso can be disabled with vdso=0

It currently includes a new gettimeofday implemention and optimized
clock_gettime(). The gettimeofday implementation is slightly faster
than the one in the old vsyscall.  clock_gettime is significantly faster
than the syscall for CLOCK_MONOTONIC and CLOCK_REALTIME.

The new calls are generally faster than the old vsyscall.

Advantages over the old x86-64 vsyscalls:
- Extensible
- Randomized
- Cleaner
- Easier to virtualize (the old static address range previously causes
overhead e.g. for Xen because it has to create special page tables for it)

Weak points:
- glibc support still to be written

The VM interface is partly based on Ingo Molnar's i386 version.

ae2c6dcf90 x86_64: various cleanups in NUMA scan node
In acpi_scan_nodes(), we immediately return -1 if acpi_numa <= 0, meaning
we haven't detected any underlying ACPI topology or we have explicitly
disabled its use from the command-line with numa=noacpi.

acpi_table_print_srat_entry() and acpi_table_parse_srat() are only
referenced within drivers/acpi/numa.c, so we can mark them as static and
remove their prototypes from the header file.

Likewise, pxm_to_node_map[] and node_to_pxm_map[] are only used within
drivers/acpi/numa.c, so we mark them as static and remove their externs
from the header file.

The automatic 'result' variable is unused in acpi_numa_init(), so it's
removed.

78b599aed6 x86_64: Don't rely on a unique IO-APIC ID
Linux 64bit only uses the IO-APIC ID as an internal cookie. In the future
there could be some cases where the IO-APIC IDs are not unique because
they share an 8 bit space with CPUs and if there are enough CPUs
it is difficult to get them that. But Linux needs the io apic ID
internally for its data structures. Assign unique IO APIC ids on
table parsing.

c0d1217202 drivers/edac: add new nmi rescan
Provides a way for NMI reported errors on x86 to notify the EDAC
subsystem pending ECC errors by writing to a software state variable.

Here's the reworked patch. I added an EDAC stub to the kernel so we can
have variables that are in the kernel even if EDAC is a module. I also
implemented the idea of using the chip driver to select error detection
mode via module parameter and eliminate the kernel compile option.
Please review/test. Thx!

Also, I only made changes to some of the chipset drivers since I am
unfamiliar with the other ones. We can add similar changes as we go.

2e1d5b8f24 x86_64: Put allocated ELF notes in read-only data segment
This changes the x86_64 linker script to use the asm-generic NOTES macro so
that ELF note sections with SHF_ALLOC set are linked into the kernel image
along with other read-only data.  The PT_NOTE also points to their location.

This paves the way for putting useful build-time information into ELF notes
that can be found easily later in a kernel memory dump.

b6a2fea393 mm: variable length argument support
Remove the arg+env limit of MAX_ARG_PAGES by copying the strings directly from
the old mm into the new mm.

We create the new mm before the binfmt code runs, and place the new stack at
the very top of the address space.  Once the binfmt code runs and figures out
where the stack should be, we move it downwards.

It is a bit peculiar in that we have one task with two mm's, one of which is
inactive.

f34e3b61f2 use the new percpu interface for shared data
Currently most of the per cpu data, which is accessed by different cpus,
has a ____cacheline_aligned_in_smp attribute.  Move all this data to the
new per cpu shared data section: .data.percpu.shared_aligned.

This will seperate the percpu data which is referenced frequently by other
cpus from the local only percpu data.

5fb7dc37dc define new percpu interface for shared data
per cpu data section contains two types of data.  One set which is
exclusively accessed by the local cpu and the other set which is per cpu,
but also shared by remote cpus.  In the current kernel, these two sets are
not clearely separated out.  This can potentially cause the same data
cacheline shared between the two sets of data, which will result in
unnecessary bouncing of the cacheline between cpus.

One way to fix the problem is to cacheline align the remotely accessed per
cpu data, both at the beginning and at the end.  Because of the padding at
both ends, this will likely cause some memory wastage and also the
interface to achieve this is not clean.

This patch:

Moves the remotely accessed per cpu data (which is currently marked
as ____cacheline_aligned_in_smp) into a different section, where all the data
elements are cacheline aligned. And as such, this differentiates the local
only data and remotely accessed data cleanly.

97ac73506c sys_fallocate() implementation on i386, x86_64 and powerpc
fallocate() is a new system call being proposed here which will allow
applications to preallocate space to any file(s) in a file system.
Each file system implementation that wants to use this feature will need
to support an inode operation called ->fallocate().
Applications can use this feature to avoid fragmentation to certain
level and thus get faster access speed. With preallocation, applications
also get a guarantee of space for particular file(s) - even if later the
the system becomes full.

Currently, glibc provides an interface called posix_fallocate() which
can be used for similar cause. Though this has the advantage of working
on all file systems, but it is quite slow (since it writes zeroes to
each block that has to be preallocated). Without a doubt, file systems
can do this more efficiently within the kernel, by implementing
the proposed fallocate() system call. It is expected that
posix_fallocate() will be modified to call this new system call first
and incase the kernel/filesystem does not implement it, it should fall
back to the current implementation of writing zeroes to the new blocks.
ToDos:
1. Implementation on other architectures (other than i386, x86_64,
   and ppc). Patches for s390(x) and ia64 are already available from
   previous posts, but it was decided that they should be added later
   once fallocate is in the mainline. Hence not including those patches
   in this take.
2. Changes to glibc,
   a) to support fallocate() system call
   b) to make posix_fallocate() and posix_fallocate64() call fallocate()

1c978b935e Inhibit NMI watchdog when Alt-SysRq-T operation is underway
On large memory configuration with not so fast CPUs the NMI watchdog is
triggered when memory addresses are being gathered and printed.  The code
paths for Alt-SysRq-t are sprinkled with touch_nmi_watchdog in various
places but not in this routine (or in the loop that utilizes this
function).  The patch has been tested for regression on large CPU+memory
configuration (128 logical CPUs + 224 GB) and 1,2,4,16-CPU sockets with
various memory sizes (1,2,4,6,20).

bcdcd8e725 Report that kernel is tainted if there was an OOPS
If the kernel OOPSed or BUGed then it probably should be considered as
tainted.  Thus, all subsequent OOPSes and SysRq dumps will report the
tainted kernel.  This saves a lot of time explaining oddities in the
calltraces.

608e261968 generic bug: use show_regs() instead of dump_stack()
The current generic bug implementation has a call to dump_stack() in case a
WARN_ON(whatever) gets hit.  Since report_bug(), which calls dump_stack(),
gets called from an exception handler we can do better: just pass the
pt_regs structure to report_bug() and pass it to show_regs() in case of a
warning.  This will give more debug informations like register contents,
etc...  In addition this avoids some pointless lines that dump_stack()
emits, since it includes a stack backtrace of the exception handler which
is of no interest in case of a warning.  E.g.  on s390 the following lines
are currently always present in a stack backtrace if dump_stack() gets
called from report_bug():

OpenVZ Linux kernel team has discovered the problem with 32bit quota tools
working on 64bit architectures.  In 2.6.10 kernel sys32_quotactl() function
was replaced by sys_quotactl() with the comment "sys_quotactl seems to be
32/64bit clean, enable it for 32bit" However this isn't right.  Look at
if_dqblk structure:

1d67953f2b Use a new CPU feature word to cover features that are spread around
Some Intel features are spread around in different CPUID leafs like 0x5,
0x6 and 0xA.  Make this feature detection code common across i386 and
x86_64.

Display Intel Dynamic Acceleration feature in /proc/cpuinfo. This feature
will be enabled automatically by current acpi-cpufreq driver.

Refer to Intel Software Developer's Manual for more details about the feature.

Thanks to hpa (H Peter Anvin) for the making the actual code detecting the
scattered features data-driven.

48d8d7ee5d x86_64 irq: use mask/unmask and proper locking in fixup_irqs()
Force irq migration path during cpu offline, is not using proper locks and
irq_chip mask/unmask routines.  This will result in some races(especially
the device generating the interrupt can see some inconsistent state,
resulting in issues like stuck irq,..).

Appended patch fixes the issue by taking proper lock and encapsulating
irq_chip set_affinity() with a mask() before and an unmask() after.

This fixes a MSI irq stuck issue reported by Darrick Wong.

There are several more general bugs in this area(irq migration in the
process context). For example,

 1. Possibility of missing edge triggered irq.
 2. Reliable method of migrating level triggered irq in the process context.

We plan to look and close these in the near future.

Eric says:
	In addition even with the fix from Suresh there is still at least one
	nasty hardware race in fixup_irqs().   However we exercise that code
	path rarely enough that we are unlikely to hit it in the real world,
	and that race seems to have existed since the code was merged.  And a
	fix for that is not coming soon as it is an open investigation area
	if we can fix irq migration to work outside of irq context or if
	we have to rework the requirements imposed by the generic cpu hotplug
	and layer on fixup_irqs().  So this may come up again.

4f84e4be53 x86_64: fix misplaced `continue' in mce.c
Background:
  When a userspace application wants to know about machine check events, it
  opens /dev/mcelog and does a read(). Usually, we found that this interface
  works well, but in some cases, when the system was taking large numbers of
  machine check exceptions, the read() would hang. The system would output a
  soft-lockup warning, and the daemon reading from /dev/mcelog would suck up
  as much of a single CPU as it could spinning in system space.

Description:
  This patch fixes this bug. In particular, there was a "continue" inside a
  timeout loop that presumably was intended to break out of the outer loop,
  but instead caused the inner loop to continue. This patch also makes the
  condition for the break-out a little more evident by changing a
  !time_before to a time_after_eq.

Result:
  The read() no longer hangs in this test case.

Testing:
  On my system, I could replicate the bug with the following command:
    # for i in `seq 15000`; do ./inject_sbe.sh; done
  where inject_sbe.sh contains commands to inject a single-bit error into the
  next memory write transaction.

Patch:
  This patch is against git f1518a088b.

- Disable CLFLUSH again; it is still broken. Always do WBINVD.
- Always flush in the i386 case, not only when there are deferred pages.

These are both brute-force inefficient fixes, to be improved
next release cycle.

The changes to i386 are a little more extensive than strictly
needed (some dead code added), but it is more similar to the x86-64 version
now and the dead code will be used soon.

12710a56cb fix sysrq-m oops
We aren't sampling for holes in memory.  Thus we encounter a section hole
with empty section map pointer for SPARSEMEM and OOPs for show_mem.  This
issue has been seen in 2.6.21, current git and current mm.  The patch below
is for mainline and mm.  It was boot tested for SPARSEMEM, current VMEMMAP
of Andy's in mm ml and DISCONTIGMEM.  A slightly different patch will be
posted to stable for 2.6.21.

Previous to commit f0a5a58aa8 memory_present
was called for node_start_pfn to node_end_pfn.  This would cover the
hole(s) with reserved pages and valid sections.  Most SPARSEMEM supported
arches do a pfn_valid check in show_mem before computing the page structure
address.

This issue was brought to my attention on IRC by Arnaldo Carvalho de Melo.

e5e3c84b70 enable interrupts in user path of page fault.
This is a minor fix, but what is currently there is essentially wrong.
In do_page_fault, if the faulting address from user code happens to be
in kernel address space (int *p = (int*)-1; p = 0xbed;)  then the
do_page_fault handler will jump over the local_irq_enable with the

  goto bad_area_nosemaphore;

But the first line there sees this is user code and goes through the
process of sending a signal to send SIGSEGV to the user task. This whole
time interrupts are disabled and the task can not be preempted by a
higher priority task.

This patch always enables interrupts in the user path of the
bad_area_nosemaphore.

On systems with huge amount of physical memory, VFS cache and memory memmap
may eat all available system memory under 4G, then the system may fail to
allocate swiotlb bounce buffer.

There was a fix for this issue in arch/x86_64/mm/numa.c, but that fix dose
not cover sparsemem model.

This patch add fix to sparsemem model by first try to allocate memmap above
4G.

d0aff6e6f4 x86_64: vsyscall time() fix
The vsyscall time() function basically returns the second portion of
xtime directly.  This however means that there is about a ticks worth of
time each second where time() will return a second value less then what
gettimeofday() does.

Additionally, this window where vtime() is behind vgettimeofday() grows
when dynticks is enabled, so its probably good to get this in before
dynticks lands.

Big thanks to Sripathi for noticing this issue and creating a test case
to work with!

This patch changes the vtime() implemenation to call vgettimeofday(),
much as syscall time() implementation calls gettimeofday().

2.6.21 stable candidate too

e8edc6e03a Detach sched.h from mm.h
First thing mm.h does is including sched.h solely for can_do_mlock() inline
function which has "current" dereference inside. By dealing with can_do_mlock()
mm.h can be detached from sched.h which is good. See below, why.

This patch
a) removes unconditional inclusion of sched.h from mm.h
b) makes can_do_mlock() normal function in mm/mlock.c
c) exports can_do_mlock() to not break compilation
d) adds sched.h inclusions back to files that were getting it indirectly.
e) adds less bloated headers to some files (asm/signal.h, jiffies.h) that were
   getting them indirectly

Net result is:
a) mm.h users would get less code to open, read, preprocess, parse, ... if
   they don't need sched.h
b) sched.h stops being dependency for significant number of files:
   on x86_64 allmodconfig touching sched.h results in recompile of 4083 files,
   after patch it's only 3744 (-8.3%).

faa8b6c3c2 Revert "ipmi: add new IPMI nmi watchdog handling"
This reverts commit f64da958df.

Andi Kleen is unhappy with the changes, and they really do not seem
worth it.  IPMI could use DIE_NMI_IPI instead of the new callback, even
though that ends up having its own set of problems too, mainly because
the IPMI code cannot really know the NMI was from IPMI or not.

Manually fix up conflicts in arch/x86_64/kernel/traps.c and
drivers/char/ipmi/ipmi_watchdog.c.

e54dc2431d [PATCH] audit signal recipients
When auditing syscalls that send signals, log the pid and security
context for each target process. Optimize the data collection by
adding a counter for signal-related rules, and avoiding allocating an
aux struct unless we have more than one target process. For process
groups, collect pid/context data in blocks of 16. Move the
audit_signal_info() hook up in check_kill_permission() so we audit
attempts where permission is denied.

8bb7844286 Add suspend-related notifications for CPU hotplug
Since nonboot CPUs are now disabled after tasks and devices have been
frozen and the CPU hotplug infrastructure is used for this purpose, we need
special CPU hotplug notifications that will help the CPU-hotplug-aware
subsystems distinguish normal CPU hotplug events from CPU hotplug events
related to a system-wide suspend or resume operation in progress.  This
patch introduces such notifications and causes them to be used during
suspend and resume transitions.  It also changes all of the
CPU-hotplug-aware subsystems to take these notifications into consideration
(for now they are handled in the same way as the corresponding "normal"
ones).

7e92b4fc34 x86, serial: convert legacy COM ports to platform devices
Make x86 COM ports into platform devices and don't probe for them
if we have PNP.

This prevents double discovery, where a device was found both by
the legacy probe and by 8250_pnp, e.g.,

    serial8250: ttyS0 at I/O 0x3f8 (irq = 4) is a 16550A
    00:02: ttyS0 at I/O 0x3f8 (irq = 4) is a 16550A

This also means IRDA devices without a UART PNP ID will no longer be
claimed by the serial driver, which might require changes in IRDA
drivers and administration.

In addition to this patch, you may need to configure a setserial init
script, e.g., /etc/init.d/setserial, so it doesn't poke legacy UART
stuff back in.  On Debian, "dpkg-reconfigure setserial" with the "kernel"
option does this.

To force the old legacy probe behavior even when we have PNPBIOS or
ACPI, load the new legacy_serial module (or build 8250 static) with
the "legacy_serial.force" option.

This patch provides a debugfs knob to turn kprobes on/off

o A new file /debug/kprobes/enabled indicates if kprobes is enabled or
  not (default enabled)
o Echoing 0 to this file will disarm all installed probes
o Any new probe registration when disabled will register the probe but
  not arm it. A message will be printed out in such a case.
o When a value 1 is echoed to the file, all probes (including ones
  registered in the intervening period) will be enabled
o Unregistration will happen irrespective of whether probes are globally
  enabled or not.
o Update Documentation/kprobes.txt to reflect these changes. While there
  also update the doc to make it current.

We are also looking at providing sysrq key support to tie to the disabling
feature provided by this patch.

Implement utimensat(2) which is an extension to futimesat(2) in that it

a) supports nano-second resolution for the timestamps
b) allows to selectively ignore the atime/mtime value
c) allows to selectively use the current time for either atime or mtime
d) supports changing the atime/mtime of a symlink itself along the lines
   of the BSD lutimes(3) functions

For this change the internally used do_utimes() functions was changed to
accept a timespec time value and an additional flags parameter.

Additionally the sys_utime function was changed to match compat_sys_utime
which already use do_utimes instead of duplicating the work.

Also, the completely missing futimensat() functionality is added.  We have
such a function in glibc but we have to resort to using /proc/self/fd/* which
not everybody likes (chroot etc).

In certain cases like when the real return address can't be found or when
the number of tracked calls to a kretprobed function is less than the
number of returns, we may not be able to find the correct return address
after processing a kretprobe.  Currently we just do a BUG_ON, but no
information is provided about the actual failing kretprobe.

Print out details of the kretprobe before calling BUG().

1eeb66a1bb move die notifier handling to common code
This patch moves the die notifier handling to common code.  Previous
various architectures had exactly the same code for it.  Note that the new
code is compiled unconditionally, this should be understood as an appel to
the other architecture maintainer to implement support for it aswell (aka
sprinkling a notify_die or two in the proper place)

arm had a notifiy_die that did something totally different, I renamed it to
arm_notify_die as part of the patch and made it static to the file it's
declared and used at.  avr32 used to pass slightly less information through
this interface and I brought it into line with the other architectures.

69331af79c Fixes and cleanups for earlyprintk aka boot console
The console subsystem already has an idea of a boot console, using the
CON_BOOT flag.  The implementation has some flaws though.  The major
problem is that presence of a boot console makes register_console() ignore
any other console devices (unless explicitly specified on the kernel
command line).

This patch fixes the console selection code to *not* consider a boot
console a full-featured one, so the first non-boot console registering will
become the default console instead.  This way the unregister call for the
boot console in the register_console() function actually triggers and the
handover from the boot console to the real console device works smoothly.
Added a printk for the handover, so you know which console device the
output goes to when the boot console stops printing messages.

The disable_early_printk() call is obsolete with that patch, explicitly
disabling the early console isn't needed any more as it works automagically
with that patch.

I've walked through the tree, dropped all disable_early_printk() instances
found below arch/ and tagged the consoles with CON_BOOT if needed.  The
code is tested on x86, sh (thanks to Paul) and mips (thanks to Ralf).

Changes to last version: Rediffed against -rc3, adapted to mips cleanups by
Ralf, fixed "udbg-immortal" cmd line arg on powerpc.

f64da958df ipmi: add new IPMI nmi watchdog handling
Convert over to the new NMI handling for getting IPMI watchdog timeouts via an
NMI.  This add config options to know if there is the ability to receive NMIs
and if it has an NMI post processing call.  Then it modifies the IPMI watchdog
to take advantage of this so that it can know if an NMI comes in.

It also adds testing that the IPMI NMI watchdog works.

74dfd666de swsusp: do not use page flags
Make swsusp use memory bitmaps instead of page flags for marking 'nosave' and
free pages.  This allows us to 'recycle' two page flags that can be used for
other purposes.  Also, the memory needed to store the bitmaps is allocated
when necessary (ie.  before the suspend) and freed after the resume which is
more reasonable.

The patch is designed to minimize the amount of changes and there are some
nice simplifications and optimizations possible on top of it.  I am going to
implement them separately in the future.

e3ebadd95c Revert "[PATCH] x86: __pa and __pa_symbol address space separation"
This was broken.  It adds complexity, for no good reason.  Rather than
separate __pa() and __pa_symbol(), we should deprecate __pa_symbol(),
and preferably __pa() too - and just use "virt_to_phys()" instead, which
is more readable and has nicer semantics.

However, right now, just undo the separation, and make __pa_symbol() be
the exact same as __pa().  That fixes the bugs this patch introduced,
and we can do the fairly obvious cleanups later.

Do the new __phys_addr() function (which is now the actual workhorse for
the unified __pa()/__pa_symbol()) as a real external function, that way
all the potential issues with compile/link-time optimizations of
constant symbol addresses go away, and we can also, if we choose to, add
more sanity-checking of the argument.

7fe3730de7 MSI: arch must connect the irq and the msi_desc
set_irq_msi() currently connects an irq_desc to an msi_desc. The archs call
it at some point in their setup routine, and then the generic code sets up the
reverse mapping from the msi_desc back to the irq.

set_irq_msi() should do both connections, making it the one and only call
required to connect an irq with it's MSI desc and vice versa.

The arch code MUST call set_irq_msi(), and it must do so only once it's sure
it's not going to fail the irq allocation.

Given that there's no need for the arch to return the irq anymore, the return
value from the arch setup routine just becomes 0 for success and anything else
for failure.

Background:
 We've found that MCEs (specifically DRAM SBEs) tend to come in bunches,
 especially when we are trying really hard to stress the system out.  The
 current MCE poller uses a static interval which does not care whether it
 has or has not found MCEs recently.

Description:
 This patch makes the MCE poller adjust the polling interval dynamically.
 If we find an MCE, poll 2x faster (down to 10 ms).  When we stop finding
 MCEs, poll 2x slower (up to check_interval seconds).  The check_interval
 tunable becomes the max polling interval.  The "Machine check events
 logged" printk() is rate limited to the check_interval, which should be
 identical behavior to the old functionality.

Result:
 If you start to take a lot of correctable errors (not exceptions), you
 log them faster and more accurately (less chance of overflowing the MCA
 registers).  If you don't take a lot of errors, you will see no change.

Alternatives:
 I considered simply reducing the polling interval to 10 ms immediately
 and keeping it there as long as we continue to find errors.  This felt a
 bit heavy handed, but does perform significantly better for the default
 check_interval of 5 minutes (we're using a few seconds when testing for
 DRAM errors).  I could be convinced to go with this, if anyone felt it
 was not too aggressive.

Testing:
 I used an error-injecting DIMM to create lots of correctable DRAM errors
 and verified that the polling interval accelerates.  The printk() only
 happens once per check_interval seconds.

Patch:
 This patch is against 2.6.21-rc7.

apic_wait_icr_idle looks like this:

The busy loop in this function would not be problematic if the
corresponding status bit in the ICR were always updated, but that does
not seem to be the case under certain crash scenarios. Kdump uses an IPI
to stop the other CPUs in the event of a crash, but when any of the
other CPUs are locked-up inside the NMI handler the CPU that sends the
IPI will end up looping forever in the ICR check, effectively
hard-locking the whole system.

Quoting from Intel's "MultiProcessor Specification" (Version 1.4), B-3:

"A local APIC unit indicates successful dispatch of an IPI by
resetting the Delivery Status bit in the Interrupt Command
Register (ICR). The operating system polls the delivery status
bit after sending an INIT or STARTUP IPI until the command has
been dispatched.

A period of 20 microseconds should be sufficient for IPI dispatch
to complete under normal operating conditions. If the IPI is not
successfully dispatched, the operating system can abort the
command. Alternatively, the operating system can retry the IPI by
writing the lower 32-bit double word of the ICR. This “time-out”
mechanism can be implemented through an external interrupt, if
interrupts are enabled on the processor, or through execution of
an instruction or time-stamp counter spin loop."

Intel's documentation suggests the implementation of a time-out
mechanism, which, by the way, is already being open-coded in some parts
of the kernel that tinker with ICR.

Create a apic_wait_icr_idle replacement that implements the time-out
mechanism and that can be used to solve the aforementioned problem.

3ebad59056 [PATCH] x86: Save and restore the fixed-range MTRRs of the BSP when suspending
Note: This patch didn'nt need an update since it's initial post.

Some BIOSes may modify fixed-range MTRRs in SMM, e.g. when they
transition the system into ACPI mode, which is entered thru an SMI,
triggered by Linux in acpi_enable().

SMIs which cause that Linux is interrupted and BIOS code is
executed (which may change e.g. fixed-range MTRRs) in SMM may
be raised by an embedded system controller which is often found
in notebooks also at other occasions.

If we would not update our copy of the fixed-range MTRRs before
suspending to RAM or to disk, restore_processor_state() would
set the fixed-range MTRRs of the BSP using old backup values
which may be outdated and this could cause the system to fail
later during resume.

This patch ensures that our copy of the fixed-range MTRRs
is updated when saving the boot processor state on suspend
to disk and suspend to RAM.

In combination with other patches this allows to fix s2ram
and s2disk on the Acer Ferrari 1000 notebook and at least
s2disk on the Acer Ferrari 5000 notebook.

AMD and Intel x86 CPU manuals state that it is the responsibility of
system software to initialize and maintain MTRR consistency across
all processors in Multi-Processing Environments.

Quote from page 188 of the AMD64 System Programming manual (Volume 2):

7.6.5 MTRRs in Multi-Processing Environments

"In multi-processing environments, the MTRRs located in all processors must
characterize memory in the same way. Generally, this means that identical
values are written to the MTRRs used by the processors." (short omission here)
"Failure to do so may result in coherency violations or loss of atomicity.
Processor implementations do not check the MTRR settings in other processors
to ensure consistency. It is the responsibility of system software to
initialize and maintain MTRR consistency across all processors."

Current Linux MTRR code already implements the above in the case that the
BIOS does not properly initialize MTRRs on the secondary processors,
but the case where the fixed-range MTRRs of the boot processor are changed
after Linux started to boot, before the initialsation of a secondary
processor, is not handled yet.

In this case, secondary processors are currently initialized by Linux
with MTRRs which the boot processor had very early, when mtrr_bp_init()
did run, but not with the MTRRs which the boot processor uses at the
time when that secondary processors is actually booted,
causing differing MTRR contents on the secondary processors.

Such situation happens on Acer Ferrari 1000 and 5000 notebooks where the
BIOS enables and sets AMD-specific IORR bits in the fixed-range MTRRs
of the boot processor when it transitions the system into ACPI mode.
The SMI handler of the BIOS does this in SMM, entered while Linux ACPI
code runs acpi_enable().

Other occasions where the SMI handler of the BIOS may change bits in
the MTRRs could occur as well. To initialize newly booted secodary
processors with the fixed-range MTRRs which the boot processor uses
at that time, this patch saves the fixed-range MTRRs of the boot
processor before new secondary processors are started. When the
secondary processors run their Linux initialisation code, their
fixed-range MTRRs will be updated with the saved fixed-range MTRRs.

If CONFIG_MTRR is not set, we define mtrr_save_state
as an empty statement because there is nothing to do.

Possible TODOs:

*) CPU-hotplugging outside of SMP suspend/resume is not yet tested
   with this patch.

*) If, even in this case, an AP never runs i386/do_boot_cpu or x86_64/cpu_up,
   then the calls to mtrr_save_state() could be replaced by calls to
   mtrr_save_fixed_ranges(NULL) and  mtrr_save_state() would not be
   needed.

   That would need either verification of the CPU-hotplug code or
   at least a test on a >2 CPU machine.

*) The MTRRs of other running processors are not yet checked at this
   time but it might be interesting to syncronize the MTTRs of all
   processors before booting. That would be an incremental patch,
   but of rather low priority since there is no machine known so
   far which would require this.

This patch touches the NMI watchdog every MAX_ORDER_NR_PAGES
to inhibit the machine from triggering an NMI while the CPUs
are locked. This situation is happening on boxes with more
than 64CPUs and 128GB of RAM when Alt-SysRq-m is performed.

It has been succesfully tested for regression on uni, 2, 4, 8
32, and 64 CPU boxes with various memory configuration.

interrupt). We can shrink it to exactly 64 bytes (1 cache line on AMD64)

I deleted an unused field : offset_base

This patch fixes one oddity in vgettimeofday(): It can returns a timeval with
tv_usec = 1000000. Maybe not a bug, but why not doing the right thing ?

2bff73830c [PATCH] x86-64: use lru instead of page->index and page->private for pgd lists management.
x86_64 currently simulates a list using the index and private fields of the
page struct.  Seems that the code was inherited from i386.  But x86_64 does
not use the slab to allocate pgds and pmds etc.  So the lru field is not
used by the slab and therefore available.

This patch uses standard list operations on page->lru to realize pgd
tracking.

6fb14755a6 [PATCH] x86: tighten kernel image page access rights
On x86-64, kernel memory freed after init can be entirely unmapped instead
of just getting 'poisoned' by overwriting with a debug pattern.

On i386 and x86-64 (under CONFIG_DEBUG_RODATA), kernel text and bug table
can also be write-protected.

Compared to the first version, this one prevents re-creating deleted
mappings in the kernel image range on x86-64, if those got removed
previously. This, together with the original changes, prevents temporarily
having inconsistent mappings when cacheability attributes are being
changed on such pages (e.g. from AGP code). While on i386 such duplicate
mappings don't exist, the same change is done there, too, both for
consistency and because checking pte_present() before using various other
pte_XXX functions is a requirement anyway. At once, i386 code gets
adjusted to use pte_huge() instead of open coding this.

8f9aeca7a0 [PATCH] x86: add command line length to boot protocol
Because the command line is increased to 2048 characters after 2.6.21, it's
not possible for boot loaders and userspace tools to determine the length
of the command line the kernel can understand.  The benefit of knowing the
length is that users can be warned if the command line size is too long
which prevents surprise if things don't work after bootup.

This patch updates the boot protocol to contain a field called
"cmdline_size" that contain the length of the command line (excluding the
terminating zero).

The patch also adds missing fields (of protocol version 2.05) to the x86_64
setup code.

8b8ca80e19 [PATCH] x86-64: configurable fake numa node sizes
Extends the numa=fake x86_64 command-line option to allow for configurable
node sizes.  These nodes can be used in conjunction with cpusets for coarse
memory resource management.

The old command-line option is still supported:
  numa=fake=32	gives 32 fake NUMA nodes, ignoring the NUMA setup of the
		actual machine.

But now you may configure your system for the node sizes of your choice:
  numa=fake=2*512,1024,2*256
		gives two 512M nodes, one 1024M node, two 256M nodes, and
		the rest of system memory to a sixth node.

The existing hash function is maintained to support the various node sizes
that are possible with this implementation.

Each node of the same size receives roughly the same amount of available
pages, regardless of any reserved memory with its address range.  The total
available pages on the system is calculated and divided by the number of equal
nodes to allocate.  These nodes are then dynamically allocated and their
borders extended until such time as their number of available pages reaches
the required size.

Configurable node sizes are recommended when used in conjunction with cpusets
for memory control because it eliminates the overhead associated with scanning
the zonelists of many smaller full nodes on page_alloc().

5a90cf205c [PATCH] x86: Log reason why TSC was marked unstable
Change mark_tsc_unstable() so it takes a string argument, which holds the
reason the TSC was marked unstable.

This is then displayed the first time mark_tsc_unstable is called.

This should help us better debug why the TSC was marked unstable on certain
systems and allow us to make sure we're not being overly paranoid when
throwing out this troublesome clocksource.

a4831e08b7 [PATCH] x86-64: Move cpu verification code to common file
o This patch moves the code to verify long mode and SSE to a common file.
  This code is now shared by trampoline.S, wakeup.S, boot/setup.S and
  boot/compressed/head.S

o So far we used to do very limited check in trampoline.S, wakeup.S and
  in 32bit entry point. Now all the entry paths are forced to do the
  exhaustive check, including SSE because verify_cpu is shared.

o I am keeping this patch as last in the x86 relocatable series because
  previous patches have got quite some amount of testing done and don't want
  to distrub that. So that if there is problem introduced by this patch, at
  least it can be easily isolated.

o Extend the bzImage protocol (same as i386) to allow bzImage loaders to
  load the protected mode kernel at non-1MB address. Now protected mode
  component is relocatable and can be loaded at non-1MB addresses.

o As of today kdump uses it to run a second kernel from a reserved memory
  area.

6a50a664ca [PATCH] x86-64: build-time checking
o X86_64 kernel should run from 2MB aligned address for two reasons.
	- Performance.
	- For relocatable kernels, page tables are updated based on difference
	  between compile time address and load time physical address.
	  This difference should be multiple of 2MB as kernel text and data
	  is mapped using 2MB pages and PMD should be pointing to a 2MB
	  aligned address. Life is simpler if both compile time and load time
	  kernel addresses are 2MB aligned.

o Flag the error at compile time if one is trying to build a kernel which
  does not meet alignment restrictions.

1ab60e0f72 [PATCH] x86-64: Relocatable Kernel Support
This patch modifies the x86_64 kernel so that it can be loaded and run
at any 2M aligned address, below 512G.  The technique used is to
compile the decompressor with -fPIC and modify it so the decompressor
is fully relocatable.  For the main kernel the page tables are
modified so the kernel remains at the same virtual address.  In
addition a variable phys_base is kept that holds the physical address
the kernel is loaded at.  __pa_symbol is modified to add that when
we take the address of a kernel symbol.

When loaded with a normal bootloader the decompressor will decompress
the kernel to 2M and it will run there.  This both ensures the
relocation code is always working, and makes it easier to use 2M
pages for the kernel and the cpu.

0dbf7028c0 [PATCH] x86: __pa and __pa_symbol address space separation
Currently __pa_symbol is for use with symbols in the kernel address
map and __pa is for use with pointers into the physical memory map.
But the code is implemented so you can usually interchange the two.

__pa which is much more common can be implemented much more cheaply
if it is it doesn't have to worry about any other kernel address
spaces.  This is especially true with a relocatable kernel as
__pa_symbol needs to peform an extra variable read to resolve
the address.

There is a third macro that is added for the vsyscall data
__pa_vsymbol for finding the physical addesses of vsyscall pages.

Most of this patch is simply sorting through the references to
__pa or __pa_symbol and using the proper one.  A little of
it is continuing to use a physical address when we have it
instead of recalculating it several times.

swapper_pgd is now NULL.  leave_mm now uses init_mm.pgd
and init_mm.pgd is initialized at boot (instead of compile time)
to the physmem virtual mapping of init_level4_pgd.  The
physical address changed.

Except for the for EMPTY_ZERO page all of the remaining references
to __pa_symbol appear to be during kernel initialization.  So this
should reduce the cost of __pa in the common case, even on a relocated
kernel.

As this is technically a semantic change we need to be on the lookout
for anything I missed.  But it works for me (tm).

o Hibernation code needs to determine the physical address associated

  don't have to be saved and restored during hibernate/resume operation.

o Move this piece of code in arch dependent section. So that architectures
  which don't have kernel text/data mapped into kernel linearly mapped
  region can come up with their own ways of determining physical addresses
  associated with a kernel text.

cfd243d4af [PATCH] x86-64: Remove the identity mapping as early as possible
With the rewrite of the SMP trampoline and the early page
allocator there is nothing that needs identity mapped pages,
once we start executing C code.

So add zap_identity_mappings into head64.c and remove
zap_low_mappings() from much later in the code.  The functions
 are subtly different thus the name change.

This also kills boot_level4_pgt which was from an earlier
attempt to move the identity mappings as early as possible,
and is now no longer needed.  Essentially I have replaced
boot_level4_pgt with trampoline_level4_pgt in trampoline.S

d8e1baf10d [PATCH] x86-64: 64bit ACPI wakeup trampoline
o Moved wakeup_level4_pgt into the wakeup routine so we can
  run the kernel above 4G.

o Now we first go to 64bit mode and continue to run from trampoline and
  then then start accessing kernel symbols and restore processor context.
  This enables us to resume even in relocatable kernel context when
  kernel might not be loaded at physical addr it has been compiled for.

o Removed the need for modifying any existing kernel page table.

o Increased the size of the wakeup routine to 8K. This is required as
  wake page tables are on trampoline itself and they got to be at 4K
  boundary, hence one page is not sufficient.

o Various cleanups. One of the main purpose of cleanups is that make
  wakeup.S as close as possible to trampoline.S.

o Following are the changes
	- Indentations for comments.
	- Changed the gdt table to compact form and to resemble the
	  one in trampoline.S
	- Take the jump to 32bit from real mode using ljmpl. Makes code
	  more readable.
	- After enabling long mode, directly take a long jump for 64bit
	  mode. No need to take an extra jump to "reach_comaptibility_mode"
	- Stack is not used after real mode. So don't load stack in
 	  32 bit mode.
	- No need to enable PGE here.
	- No need to do extra EFER read, anyway we trash the read contents.
	- No need to enable system call (EFER_SCE). Anyway it will be
	  enabled when original EFER is restored.
	- No need to set MP, ET, NE, WP, AM bits in cr0. Very soon we will
  	  reload the original cr0 while restroing the processor state.

This modifies the SMP trampoline and all of the associated code so
it can jump to a 64bit kernel loaded at an arbitrary address.

The dependencies on having an idenetity mapped page in the kernel
page tables for SMP bootup have all been removed.

In addition the trampoline has been modified to verify
that long mode is supported.  Asking if long mode is implemented is
down right silly but we have traditionally had some of these checks,
and they can't hurt anything.  So when the totally ludicrous happens
we just might handle it correctly.

Move __KERNEL32_CS up into the unused gdt entry.  __KERNEL32_CS is
used when entering the kernel so putting it first is useful when
trying to keep boot gdt sizes to a minimum.

Set the accessed bit on all gdt entries.  We don't care
so there is no need for the cpu to burn the extra cycles,
and it potentially allows the pages to be immutable.  Plus
it is confusing when debugging and your gdt entries mysteriously
change.

- Merge physmem_pgt and ident_pgt, removing physmem_pgt.  The merge
  is broken as soon as mm/init.c:init_memory_mapping is run.
- As physmem_pgt is gone don't export it in pgtable.h.
- Use defines from pgtable.h for page permissions.
- Fix the physical memory identity mapping so it is at the correct
  address.
- Remove the physical memory mapping from wakeup_level4_pgt it
  is at the wrong address so we can't possibly be usinging it.
- Simply NEXT_PAGE the work to calculate the phys_ alias
  of the labels was very cool.  Unfortuantely it was a brittle
  special purpose hack that makes maitenance more difficult.
  Instead just use label - __START_KERNEL_map like we do
  everywhere else in assembly.

dafe41ee3a [PATCH] x86-64: Kill temp boot pmds
Early in the boot process we need the ability to set
up temporary mappings, before our normal mechanisms are
initialized.  Currently this is used to map pages that
are part of the page tables we are building and pages
during the dmi scan.

The core problem is that we are using the user portion of
the page tables to implement this.  Which means that while
this mechanism is active we cannot catch NULL pointer dereferences
and we deviate from the normal ways of handling things.

In this patch I modify early_ioremap to map pages into
the kernel portion of address space, roughly where
we will later put modules, and I make the discovery of
which addresses we can use dynamic which removes all
kinds of static limits and remove the dependencies
on implementation details between different parts of the code.

Now alloc_low_page() and unmap_low_page() use
early_iomap() and early_iounmap() to allocate/map and
unmap a page.

This patch against 2.6.20-git14 makes the NMI watchdog use PERFSEL1/PERFCTR1
instead of PERFSEL0/PERFCTR0 on processors supporting Intel architectural
perfmon, such as Intel Core 2. Although all PMU events can work on
both counters, the Precise Event-Based Sampling (PEBS) requires that the
event be in PERFCTR0 to work correctly (see section 18.14.4.1 in the
IA32 SDM Vol 3b). This versions has 3 chunks compared to previous where
we had missed on check.

Changelog:
        - make the x86-64 NMI watchdog use PERFSEL1/PERFCTR1 instead of PERFSEL0/PERFCTR0
          on processors supporting the Intel architectural perfmon (e.g. Core 2 Duo).
          This allows PEBS to work when the NMI watchdog is active.

a userspace fault or a kernelspace fault which will result in the
immediate death of the process.  They should not be filled in as a
result of a kernelspace fault which can be fixed up.

Otherwise, if the process is handling SIGSEGV and examining the fault
information, this can result in the kernel space fault trashing the
previously stored fault information if it arrives between the
userspace fault happening and the SIGSEGV being delivered to the process.

Synchronize i386's smp_send_stop() with x86-64's in only try-locking
the call lock to prevent deadlocks when called from panic().
In both version, disable interrupts before clearing the CPU off the
online map to eliminate races with IRQ handlers inspecting this map.
Also in both versions, save/restore interrupts rather than disabling/
enabling them.
On x86-64, eliminate one function used here by folding it into its
single caller, convert to static, and rename for consistency with i386
(lkcd may like this).

mode, broadcasting is limited to within the cluster only, and chipsets have

f18d397e6a [PATCH] x86-64: optimize & fix APIC mode setup
Fix a couple of inconsistencies/problems I found while reviewing the x86_64
genapic code (when I was chasing mysterious eth0 timeouts that would only
trigger if CPU_HOTPLUG is enabled):

 - AMD systems defaulted to the slower flat-physical mode instead
   of the flat-logical mode. The only restriction on AMD systems
   is that they should not use clustered APIC mode.

 - removed the CPU hotplug hacks, switching the default for small
   systems back from phys-flat to logical-flat. The switching to logical
   flat mode on small systems fixed sporadic ethernet driver timeouts i
   was getting on a dual-core Athlon64 system:

    NETDEV WATCHDOG: eth0: transmit timed out

 - The use of '<= 8' was a bug by itself (the valid APIC ids
   for logical flat mode go from 0 to 7, not 0 to 8). The new logic
   is to use logical flat mode on both AMD and Intel systems, and
   to only switch to physical mode when logical mode cannot be used.
   If CPU hotplug is racy wrt. APIC shutdown then CPU hotplug needs
   fixing, not the whole IRQ system be made inconsistent and slowed
   down.

 - minor cleanups: simplified some code constructs

build & booted on a couple of AMD and Intel SMP systems.

8cdfb29c0c libata/IDE: remove combined mode quirk
Both old-IDE and libata should be able handle all controllers and
devices found using normal resource reservation methods.

This eliminates the awful, low-performing split-driver configuration
where old-IDE drove the PATA portion of a PCI device, in PIO-only mode,
and libata drove the SATA portion of the /same/ PCI device, in DMA mode.
Typically vendors would ship SATA hard drive / PATA optical
configuration, which would lend itself to slow (PIO-only) CD-ROM
performance.

For Intel users running in combined mode, it is now wholly dependent on
your driver choice (potentially link order, if you compile both drivers
in) whether old-IDE or libata will drive your hardware.

In either case, you will get full performance from both SATA and PATA
ports now, without having to pass a kernel command line parameter.

90767bd13f [PATCH] x86-64: Always flush all pages in change_page_attr
change_page_attr on x86-64 only flushed the TLB for pages that got
reverted. That's not correct: it has to be flushed in all cases.

This bug was added in some earlier changes.

Just flush all pages for now.

This could be done more efficiently, but for this late in the release
this seem to be the best fix.

This patches fixes the silent data corruption problems being seen using the
GART iommu where 4kB of data where incorrect (seen mostly on Nvidia CK804
systems).  This fix, to mark the memory regin the GART PTEs reside on as
uncacheable, also brings the code in line with the AGP specification.

Due to an over aggressive optimizer gcc 4.2 cannot optimize away _proxy_pda
in all cases (counter intuitive, but true).  This breaks loading of some
modules.

The earlier workaround to just export a dummy symbol didn't work unfortunately
because the module code ignores exports with 0 value.

Make it 1 instead.

436ce71638 [PATCH] Revert "swsusp: disable nonboot CPUs before entering platform suspend"
This reverts commit 94985134b7 and
insteads removes the WARN_ON() that caused that commit in the first
place.

The problem is that we call disable_nonboot_cpus() in swsusp before
powering down the system in order to avoid triggering the WARN_ON()
in arch/x86_64/kernel/acpi/sleep.c:init_low_mapping() and this doesn't
work well on Thomas' system.

So instead, remove the WARN_ON() in arch/x86_64/kernel/acpi/sleep.c:
init_low_mapping(), which triggers every time during the suspend to disk
in the platform mode, as the potential problem it is related to doesn't
seem to occur in practice.

[ I think we might want to disallow the case of multiple users of that
  mm, or something.  Normally, playing with the current process page
  tables on the current CPU should be fine as long as we don't have
  other threads using those tables at the same time..

  Anyway, not pretty, but better than the warning or the lockup - Linus ]

f33bc55c47 [PATCH] x86_64: avoid sending LOCAL_TIMER_VECTOR IPI to itself
Ray Lee reported, that on an UP kernel with "noapic" command line option
set, the box locks hard during boot.

Adding some debug printks revealed, that the last action on the box
before stalling was "Send IPI" - a debug printk which was put into
smp_send_timer_broadcast_ipi().

It seems that send_IPI_mask(mask, LOCAL_TIMER_VECTOR) fails when
"noapic" is set on the command line on an UP kernel.

Aside of that it does not make much sense to trigger an interrupt
instead of calling the function directly on the CPU which gets the
PIT/HPET interrupt in case of broadcasting.

fe69933652 [PATCH] ACPI: repair nvidia early quirk breakage on x86_64
x86_64 nvidia_bugs() broke when we bailed out on not finding the HPET.
However, the quirk works by checking for _not_ finding the HPET...

Delete the nvidia_hpet_detected flag and simply test for
not finding the HPET, which is simple to do now that
acpi_table_parse returns 1 on failure.

47c51dff4a [PATCH] x86_64: fix 2.6.18 regression - PTRACE_OLDSETOPTIONS should be accepted
Also PTRACE_OLDSETOPTIONS should be accepted, as done by kernel/ptrace.c
and forced by binary compatibility.  UML/32bit breaks because of this -
since it is wise enough to use PTRACE_OLDSETOPTIONS to be binary compatible
with 2.4 host kernels.

Until 2.6.17 (commit f0f2d6536e) we had:

       default:
                return sys_ptrace(request, pid, addr, data);

Instead here we have:
        case PTRACE_GET_THREAD_AREA:
	case ...:
                return sys_ptrace(request, pid, addr, data);

        default:
                return -EINVAL;

This change was a style change - when a case is added, it must be
explicitly tested this way.  In this case, not enough testing was done.

d04f41e353 [PATCH] CPU hotplug: call check_tsc_sync_source() with irqs off
check_tsc_sync_source() depends on being called with irqs disabled (it
checks whether the TSC is coherent across two specific CPUs). This is
incidentally true during bootup, but not during cpu hotplug __cpu_up().
This got found via smp_processor_id() debugging.

disable irqs explicitly and remove the unconditional enabling of
interrupts. Add touch_nmi_watchdog() to the cpu_online_map busy loop.

this bug is present both on i386 and on x86_64.
