Bytes 464..511 in the current 512byte layout of fxsave/fxrstor
frame, are reserved for SW usage. On cpu's supporting xsave/xrstor, these bytes
are used to extended the fpstate pointer in the sigcontext, which now includes
the extended state information along with fpstate information.

This patch is the result of an automatic script that consolidates the
format of all the headers in include/asm-x86/.
The format:
1. No leading underscore. Names with leading underscores are reserved.
2. Pathname components are separated by two underscores. So we can
distinguish between mm_types.h and mm/types.h.
3. Everything except letters and numbers are turned into single
underscores.

Jakub Jelinek reported that some user-space code that relies on
kernel headers has built dependency on the sigcontext->eip/rip
register names - which have been unified in commit:

x86: use generic register names in struct sigcontext
so give the old layout to user-space. This is not particularly
pretty, but it's an ABI so there's no danger of the two definitions
getting out of sync.

Switch struct sigcontext (defined in <asm/sigcontext*.h>) to using
register names withut e- or r-prefixes for both 32- and 64-bit x86.
This is intended as a preliminary step in unifying this code between
architectures.
