On plug-in of my USB-C device, its USB_SS_PORT_LS_SS_INACTIVE
link state bit is set. Greping all the kernel for this bit shows
that the port status requests a warm-reset this way.
This just happens, if its the only device on the root hub, the hub
therefore resumes and the HCDs status_urb isn't yet available.
If a warm-reset request is detected, this sets the hubs event_bits,
which will prevent any auto-suspend and allows the hubs workqueue
to warm-reset the port later in port_event.

On my Yeeloong 8089, I noticed the machine fails to shutdown
properly, and often, the function mach_prepare_reboot() is
unexpectedly executed, thus the machine reboots instead. A
wait loop is needed to ensure the system is in a well-defined
state before going down.
In commit 997e93d4df16 ("MIPS: Hang more efficiently on
halt/powerdown/restart"), a general superset of the wait loop for all
platforms is already provided, so we don't need to implement our own.
This commit simply removes the unreachable() compiler marco after
mach_prepare_reboot(), thus allowing the execution of machine_hang().
My test shows that the machine is now able to shutdown successfully.
Please note that there are two different bugs preventing the machine
from shutting down, another work-in-progress commit is needed to
fix a lockup in cpufreq / i8259 driver, please read Reference, this
commit does not fix that bug.

Bugzilla: 1671930
Emulation of certain instructions (VMXON, VMCLEAR, VMPTRLD, VMWRITE with
memory operand, INVEPT, INVVPID) can incorrectly inject a page fault
when passed an operand that points to an MMIO address. The page fault
will use uninitialized kernel stack memory as the CR2 and error code.
The right behavior would be to abort the VM with a KVM_EXIT_INTERNAL_ERROR
exit to userspace; however, it is not an easy fix, so for now just
ensure that the error code and CR2 are zero.

kvm_ioctl_create_device() does the following:
1. creates a device that holds a reference to the VM object (with a borrowed
reference, the VM's refcount has not been bumped yet)
2. initializes the device
3. transfers the reference to the device to the caller's file descriptor table
4. calls kvm_get_kvm() to turn the borrowed reference to the VM into a real
reference
The ownership transfer in step 3 must not happen before the reference to the VM
becomes a proper, non-borrowed reference, which only happens in step 4.
After step 3, an attacker can close the file descriptor and drop the borrowed
reference, which can cause the refcount of the kvm object to drop to zero.
This means that we need to grab a reference for the device before
anon_inode_getfd(), otherwise the VM can disappear from under us.

Recently syzkaller was able to create unkillablle processes by
creating a timer that is delivered as a thread local signal on SIGHUP,
and receiving SIGHUP SA_NODEFERER. Ultimately causing a loop failing
to deliver SIGHUP but always trying.
When the stack overflows delivery of SIGHUP fails and force_sigsegv is
called. Unfortunately because SIGSEGV is numerically higher than
SIGHUP next_signal tries again to deliver a SIGHUP.
From a quality of implementation standpoint attempting to deliver the
timer SIGHUP signal is wrong. We should attempt to deliver the
synchronous SIGSEGV signal we just forced.
We can make that happening in a fairly straight forward manner by
instead of just looking at the signal number we also look at the
si_code. In particular for exceptions (aka synchronous signals) the
si_code is always greater than 0.
That still has the potential to pick up a number of asynchronous
signals as in a few cases the same si_codes that are used
for synchronous signals are also used for asynchronous signals,
and SI_KERNEL is also included in the list of possible si_codes.
Still the heuristic is much better and timer signals are definitely
excluded. Which is enough to prevent all known ways for someone
sending a process signals fast enough to cause unexpected and
arguably incorrect behavior.

Recently syzkaller was able to create unkillablle processes by
creating a timer that is delivered as a thread local signal on SIGHUP,
and receiving SIGHUP SA_NODEFERER. Ultimately causing a loop
failing to deliver SIGHUP but always trying.
Upon examination it turns out part of the problem is actually most of
the solution. Since 2.5 signal delivery has found all fatal signals,
marked the signal group for death, and queued SIGKILL in every threads
thread queue relying on signal->group_exit_code to preserve the
information of which was the actual fatal signal.
The conversion of all fatal signals to SIGKILL results in the
synchronous signal heuristic in next_signal kicking in and preferring
SIGHUP to SIGKILL. Which is especially problematic as all
fatal signals have already been transformed into SIGKILL.
Instead of dequeueing signals and depending upon SIGKILL to
be the first signal dequeued, first test if the signal group
has already been marked for death. This guarantees that
nothing in the signal queue can prevent a process that needs
to exit from exiting.

In case when some EP IN is frequently reused, i.e. enabled/disabled by
function driver. It is required to clear TxFIFO number field in DIEPCTL
register before setting new number. Otherwise there is probability to
have same TxFIFO number for different EP's because of OR operator.

A collection of a few small fixes. The most significant one is the
fix for the possible race at loading HD-audio drivers. This has been
present for long time and surfaced only in a rare occasion, but
finally spotted out.
The rest are usual device-specific fixes for HD-audio and USB-audio.

Cadence has designed a D-PHY that can be used by the, currently in tree,
DSI bridge (DRM), CSI Transceiver and CSI Receiver (v4l2) drivers.
Only the DSI driver has an ad-hoc driver for that phy at the moment, while
the v4l2 drivers are completely missing any phy support. In order to make
that phy support available to all these drivers, without having to
duplicate that code three times, let's create a generic phy framework
driver.

The Cadence D-PHY bindings was defined as part of the DSI block so far.
However, since it's now going to be a separate driver, we need to move the
binding to a file of its own.

The lanes parameter is not solely about the number of lanes, but it also
carries the fact that those are the first lanes in use during the
transmission.
It was implicit so far, so make sure it's explicit now.

Marvell Armada 3700 SoC has two USB controllers, each of them being
wired to an internal UTMI PHY. Add a driver to control them.

Current file describe COMPHY bindings for the IP available on the
CP110 of Armada 7k/8k. Bindings are very close (and serve the same
purpose) as the new Armada 3700 COMPHY driver so update this file to
describe both. Also add an example of how to use this second
compatible (same as for the ESPRESSObin).
While doing so, enhance a bit the file by adding upper case where
needed.

Add a driver to support COMPHY, a hardware block providing shared
serdes PHYs on Marvell Armada 3700. This driver uses SMC calls and
rely on having an up-to-date firmware.
SATA, PCie and USB3 host mode have been tested successfully with an
ESPRESSObin. (HS)SGMII mode cannot be tested with this platform.
Evan worked on the original driver structure and Grzegorz on the SMC
calls rework. The structure of this driver has been copied from
Antoine Tenart work on CP110 COMPHY driver.

So far the PHY ->xlate() callback was checking if the port was
"invalid" before continuing, meaning that the port has not been used
yet. This check is not correct as there is no opposite call to
->xlate() once the PHY is released by the user and the port will
remain "valid" after the first phy_get()/phy_put() calls. Hence, if
this driver is built as a module, inserted, removed and inserted
again, the PHY will appear busy and the second probe will fail.
To fix this, just drop the faulty check and instead verify that the
port number is valid (ie. in the possible range).
