Embed the video_device struct to simplify the error handling and in
order to (eventually) get rid of video_device_alloc/release.

VIDIOC_DBG_G_CHIP_IDENT has been replaced by VIDIOC_DBG_G_CHIP_INFO. Remove
g_chip_ident support from bridge drivers since it is no longer needed.
This patch takes care of all the trivial cases.

Write-only ioctls should have a const argument in the ioctl op.
Do this conversion for vidioc_s_audio.
Adding const for write-only ioctls was decided during the 2012 Media Workshop.
