John SmithandClaude 09443939e6 fix(server): server-seeded rolls, role gates on stateless routes, migration parity (iteration 6) ...

Client-supplied seeds on check/save rejected 422 SEED_NOT_PERMITTED except
GM/service principals (determinism opt-in). Spectators blocked from
actions/spatial/map-gen; scripts GM+service-only. ledger_sequence + unique
index added to 02_*.sql matching runtime DDL; id-space reconciliation
documented. Five new tests red-first; cargo 247/18, clippy clean.
