John Doe 587d2dd1d6 Security remediation: Liberation plugin auto-approve fix ...

- SEC-011/C4: Fixed auto-approve default values
  - Changed ?? true fallbacks to ?? false for approvalBypassEnabled and autoApprove
  - Config files already set these to false, but source code defaulted to true
  - Prevents unauthorized auto-approval of plugin operations

- Updated openclaw-liberation-plugin config files and source code
- Cleaned up clawbridge-dashboard README (removed 332 lines)
- Added REMEDIATION_LOG.md documenting all changes
