John Smith 17fa72dd7b fix: remediate PR review findings (CI build, doctor name, shell deny, sanitize, effect-free schemas) 
- CI: build plugin before E2E in e2e and e2e-compat (dist/ gitignored)
- commands: /rea-doctor always references constant rea_readiness
- agents: deny shell for rea-analyst/rea-auditor/rea-runtime
- options/agents/readiness: sanitize server namespace to host
  derivation; reject empty/whitespace server
- readiness: replace effect Schema with plain JSON Schema (no new deps)
- tests+README synced; check clean, unit 82 pass

John Smith 661786b2d7 feat(T09): slash commands rea rea-doctor rea-report 
T09: register rea (switch to rea agent + staged investigation), rea-doctor (server_readiness check), and rea-report (evidence report + rea-auditor check) via ctx.command.transform. Wire into plugin setup. T09 integration test proves live in-process registration; T04 setup stub gains the command.transform no-op (same pattern as T05/T07). No shadowing of the six rea:<prompt> MCP prompt commands.
