John Smith 56fde1ad02 feat(server): warn when certificate key encryption is disabled 
- log a one-time startup warning when DROP_CERTIFICATE_KEY is unset so
  plaintext certificate key storage is not silent
- export certificateKeyConfigured with a unit test
- document the variable and the encryption-at-rest behavior in the server
  reference docs
