Continuing nsIPresContext deCOMtamination (bug 229371). Remove GetImageLoadFlags (unused). Make failure to fetch the LookAndFeel service cause Init() to fail, don't null check it after that, and inlined the getter. Move IOService caching to nsImageFrame, the only user of it. r+sr=bzbarsky.

Eliminate nsIStyleSet as an XPCOM interface, making the implementation internal to layout. Frame constructor ownership moves to the pres shell, and the style set methods that forward to the frame constructor are gone. Rewrote style set sheet add/remove to reduce code size. Fixing bug 124412. Making nsXULDocument inherit nsXMLDocument to avoid the need to duplicate a large amount of code in nsXULDocument. Also removing nsMarkupDocument and nsIDOMEventCapturer that are no longer needed.
