cd9ff48a56 Bug 550944 - If a stream is alive at instance destruction, make sure we fire NPP_DestroyStream and NPP_URLNotify correctly. Also make the new-stream functions (NPN_GetURL etc) fail while we are destroying a plugin instance, r=josh
