31f1898810 Bug 300030: Move intrinsic width computation out of nsIFrame::Reflow and into its own methods on nsIFrame. Replace reflow reasons, types, and commands with dirty bits/notifications. Thanks to bzbarsky for almost all of the HTML form controls (mozilla/layout/forms) changes, and many others for help testing and patching. dfe3513814 Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. 5a6317b8a5 Bug 88413: Remove |GetUnicode()| from nsString (and replace it with |get()|). r=dbaron, rs=scc. 
This removes all call-sites I can currently fix. Tomorrow I'll try to get someone to checkin my changes to security/ and I'll get some help with the Netscape side of things.

nsString::GetUnicode()'s final death-blow will be dealt soon. Please keep this in mind as you add new code :-)
