This removes all call-sites I can currently fix. Tomorrow I'll try to get someone to checkin my changes to security/ and I'll get some help with the Netscape side of things.
nsString::GetUnicode()'s final death-blow will be dealt soon. Please keep this in mind as you add new code :-)

Bug 77834: Fix problem where form control state is being restored to the wrong form control or not restored at all when going back and forth through session history. The problem was that the hash key we used was the content ID, which was not consistent from page load to page load. The fix was to key instead by index of form in document / index of control in form, with logic to fall back to content ID if we are unable to do this. r=nisheeth@netscape.com, sr=jst@netscape.com, a=drivers@mozilla.org (blizzard@mozilla.org)

This checkin enables mozilla to support the printing of selection, the printing of page ranges, and

the printing of headers and footers.
Printing of selection is implemented by the frames figuring out if they are in the selection and painting
if they or not they they don't paint. This also only allows the printing of the first page of
selections, alothough it is well documented where this is implemeted so it can be removed.

Fix leak on hovering over a scrollbar and re-fix similar leak for text inputs (bug 42895) by adding a "general" method for unrooting anonymous content created through nsIAnonymousContentCreator, plus a hack to deal with the scrollbars on the root element. Changing the way content elements store their name information, previously the elements generally stored a name atom and a namespace ID, now they store a pointer to a shared structure containing the name atom, the prefix atom and the namespace ID. This structure is shared between nodes with unique names (ie same name, prefix and ns ID) within a document. Documents now hold a hash table of the names in the document. The changes to mozilla/rdf are not reviewed but they are approved by waterson@netscape.com and the changes to mozilla/layout are reviewed by buster@netscape.com.
