a58105db04 Added a new InitializeMacToolbox() to NSStdLib (declared in macstdlibextras.h) and exported it. This makes all the Mac Toolbox Init calls, and also (if DEBUG) initializes SIOUX without menus. Safe to call this multiple times (it has a static boolean). Removed toolbox initialization from constructor of nsAppShell, and called InitializeMacToolbox() instead. 65038e9551 Removing static makefiles as a part of the transition to Autoconf. I have taken great 
care to only remove makefiles affailiated with building nglayout. I have also verified that
the build at this point in time succesfully builds with autoconf.

If you must recover one of these makefiles, you can use the static tag STATIC_MAKEFILES.

f5bbd55233 Fixed the implementation of nsINetSupport. Before it was totally busted since it assumed that nsString could return a const char *buffer (which it cant).
