Replace the BodyFixupRule with code in nsCSSRendering to fix all the problems with dynamic style changes and backgrounds on the root element and the BODY element. Reduce the performance cost of (threadsafely) refcounting many atoms by creating a second implementation of nsIAtom (derived from the first) that does not refcount and gets destroyed at XPCOM shutdown. Normal atoms are converted to permanent ones without loss of pointer identity when a permanent atom is requested and a normal one already exists. b=92141 r=waterson sr=brendan

r=dbaron, sr=dougt. 97667. nsIInterfaceRequestor is now frozen. nsIInterfaceRequestorUtils.h is now required if you want to use do_GetInterface() for convenience. The two were split because the stuff that is now in nsIInterfaceRequestorUtils.h wasn't ready for freezing. Also, .idl files should just contain interface defs anyway.
