c20f157140 Major rework of how items are added, removed and selected (while being added and removed) with script 
Now all the changes are performed, but no reflows are issued until the timer fires, which would be
after the script is done executing. The selection that take place during the script execution are
also cached and done afterward.

1ac7a3715e Bug 77834: Fix problem where form control state is being restored to the wrong form control or not restored at all when going back and forth through session history. The problem was that the hash key we used was the content ID, which was not consistent from page load to page load. 706bdfa5fd When clicking away from the dropdown, it wasn't resetting itself correctly. 
If only the mouse was being used for selection, then it needs to reset itself to the state before dropping down
if arrow keys had been used then it needs to keep that selection
The combobox chaches the current selection, so when arrow keys are used it has the correct selection
when the mouse is used it holds the old selection. So therefore, we can compare against it
to determine what to do.

965ad980fb Focus rect is now drawn and erased directly via C++ code. Focus comes from "SetFocus" being set 
on the content node which calls the frame. The frame is notified of Blur via thecontent node
watching for NS_CONTENT_BLUR events.
The rule we are removing from html.css is responsible for drawing in the focus,
but it causes the combobox, the dropdown and all the options to have their style reresolved.

edbb56773e Fix leak on hovering over a scrollbar and re-fix similar leak for text inputs (bug 42895) by adding a "general" method for unrooting anonymous content created through nsIAnonymousContentCreator, plus a hack to deal with the scrollbars on the root element. Some of the other SetDocumentForAnonymousContent methods *may* need to be implemented. 42afe24660 using correct usesage pattern for getting and seeting the FrameState 
Added in my new reflow code with an ifdef so I can work on it and fix other bugs
fixed the checking of return codes for getHTMLAttr
removed ShouldScrollFrameIntoView, the presshell can now do this.
also commented out the call to Position the Dropdown

ua.css - added rules for select to differentiate between comboboxes and listboxes.
Added style rules to more closely match the XPTOOLKIT XPWidgets look.
removed the following :-moz-combobox-text, -moz-combobox-textselected
nsIFormControlFrame.h - Added SetSuggestedSize method.
nsButtonControlFrame - Implemented SetSuggestedSize.
nsCSSFrameConstructor.cpp - Rewrote ConstructSelectFrame.

664a8f2a93 bug #5993. Removed references to nsIWidget in form HTML content elements. the form elements 
now go through the nsFormControlFrame interface instead. This removes the dependency between
form element content and nsIWidget.

e18026d96e bug #3547- Modified nsFileControlFrame.cpp to call nsFormFrame::AddFormControlFrame. 
bug #5589. Selection area is now calculated using the inherited Reflow rather
than using the calculations used for the native listbox control.
Fixed varous other rendering bugs related to frame-based listboxes.
bug #2797. GetSize is no longer needed in nsListControlFrame and it has been removed.
Fixed frame-based combobox nsComboboxFrame so that will set it's label to the
first option in the drop-down list if a default value has not been specified.
Added optgroup style rules to ua.css. Opt groups now render, but there are remaining
problems with selection.
Modified nsButtonControlFrame::Reflow to call nsFormFrame::AddFormControlFrame.
This fixes a bug where Gfx-rendered submit button's were not working.
Refactored code in nsListControlFrame in preparation for supporting option groups.
Refactored selection code into SingleSelect and MultiSelect paths.
Removed unused code in nsListControlFrame.
Removed mSelectedFrame member variable. It is no longer needed.
Fixed nsListControlFrame::SetProperty so that it will toggle the selection in a
single select listbox.

Added -moz-dropdown-btn-out and -moz-dropdown-btn-pressed, -moz-dropdown-list rules.
Modified nsListControlFrame.cpp to calculate the width of the dropdown list for combo boxes correctly
Added nsListControlFrame::GetProperty and SetProperty methods.
Modified nsCSSFrameConstructor::ConstructSelectFrame to setup combobox.

204a6670eb 1) Implemented regular button and html4 button with a button renderer. 
2) Fixed ProgressMeter to update correctly when attributes change
3) Fixed sample8.html so that it does not over ride the borders of the HTML4 button this
   messed up the active, hover, and disabled states.
