Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. f384166efa Create a mozAutoDocUpdate class to handle automatically calling 
BeginUpdate/EndUpdate on documents.  Use this in a bunch of places to make sure
that EndUpdate is called in the face of early returns.  Bug 131775, also fixes
