Bug 300030: Move intrinsic width computation out of nsIFrame::Reflow and into its own methods on nsIFrame. Replace reflow reasons, types, and commands with dirty bits/notifications. Thanks to bzbarsky for almost all of the HTML form controls (mozilla/layout/forms) changes, and many others for help testing and patching. For detailed commit logs, see REFLOW_YYYYMMDD_BRANCH, where YYYYMMDD is one of 20061031, 20060830, 20060603, 20060302, 20060119, 20051011, 20050804, 20050429, 20050315, 20050111, and 20041213.

Rename {nsIDocument,nsIDocumentObserver,nsIFrame}::ContentChanged to CharacterDataChanged, since that's the only thing it's used for. Replace its nsISupports* parameter with a PRBool, since we don't need to allocate an object to pass a boolean parameter. Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. This checkin enables mozilla to support the printing of selection, the printing of page ranges, and

Printing of selection is implemented by the frames figuring out if they are in the selection and painting
if they or not they they don't paint. This also only allows the printing of the first page of
selections, alothough it is well documented where this is implemeted so it can be removed.

Fix 32199 (visibility:collapse is not being interpreted as hidden on block and inline level elements) and parts of 21701 (misuse of style attr mVisible).
In the following files, replacing disp->mVisible with disp->IsVisible() is a BUG FIX: nsBlockFrame, nsBulletFrame, nsHRFrame, nsHTMLContainerFrame, nsImageFrame, nsTextFrame.
In all the other files, replacing disp->mVisible with disp->IsVisible() or disp->IsVisibleOrCollapsed() is merely a SYNTACTICAL CHANGE. The respective owners will later review the use of IsVisibleOrCollapsed() and in some cases replace it with IsVisible(). See bug 21701.
