0b6ea74a27 Bug 558390 - Quake Live crashes because it implements NP_Initialize using cdecl, instead of the correct stdcall. Force the calling function to use a frame pointer so that the stack pointer gets reset correctly before we return, r=jrmuizelaar, landing in mozilla-central for bake time pending review from josh or jst

To this end, the classes nsPluginStreamInfo and nsPluginStreamListenerPeer have been unified: the nsNPAPIPluginStreamListener holds the stream listener peer alive so that additional range requests can be made on it. In addition the nsNPAPIPluginStreamListener holds *itself* alive for NP_SEEK streams, and only calls releases itself/calls CleanUpStream when the plugin explicitly calls NPN_DestroyStream or when the associated plugin instance is being destroyed. r=josh
