Also deletes a bunch of dead code, and makes some variables local
variables instead of member variables since they are only used in
a single function and don't need to be persisted.

003c8f3047 Bug 867758 - Synchronize the first time we call RequestRefresh() on an image with the time we call it on images with the same first frame delay, and set the animation start time on those images to the refresh driver's time stamp for accuracy. r=seth
