Test Results Only: Include size in implicitly sized arrays (change to unsized).

This effects 60 tests, in a trivial way, but is critical to factor out this
noise from future planned changes.

GLSL output: Removed fixed-size buffer; fixes #769 .

Makes some white-space differences in most output, plus a few cases
where more could have been put out but was cut short by the previous
fix-sized buffer.

Front-end: Fix missing tessellation shader built-in qualifiers.

The gl_in array has a special path due to context-specific
gl_MaxPatchVertices, making the code out of order for tagging built-ins.
This commit moves the tagging to the correct location.
This also fixes issue #80 .

Additional semantic checking for tessellation shaders and related clean up:
...
- non-arrayed geometry inputs and tessellation-control outputs
- include user blocks in i/o array resizing based on primitive type
- things belonging only to 'in' or 'out' are verified only on 'in' or 'out'
- invocation number changing
- move shader-scoped qualifier checking into its class
- remove an incidental union of two levels of the node hierarchy in the yacc stack
- remove layout(location=) default tracking when setting on a variable
