This PR is based on this issue: https://github.com/sst/opencode/issues/4334

I created a PR in the models.dev repository to add Azure Cognitive Services as a provider (https://github.com/sst/models.dev/pull/393).
I was able to use the ai-sdk/azure and used the baseURL option to point to cognitiveservices.azure.com instead of openai.azure.com.

Although there is a dependency on models.dev for others to use this, I was able to test it by generating the JSON and testing locally, and it works.

I updated documentation as well, for users to know how to set this up.
