I think this might have been since v0.4 started with the mode/agent merge, but when I'm using the Plan Agent to initiate a new session no title gets generated, If I start with Build Agent I do get a title generated.

Ok, i can't either now. Maybe it was a hiccup in the small model? I only noticed it when I switched to 0.4 with gpt-5 yesterday, i had this happen

The New sessions are full work sessions that started in plan agent, the ones with titles start in build agent

If I had to guess openai servers were getting hammered and the title gens were just taking way too long

probably some timeout error for ai sdk, tho I think some requests can hang for literal minutes
