[openai-codex-plugin] 400 error: {"error":{"message":"No tool call found for function call output with call_id call_HTwVL96Xq7Y4FTxLZLkoqFK2.","type":"invalid_request_error","param":"input","code":null,"status":400}}

OpenCode version

1.0.132

Steps to reproduce

I ask opencode to create a file called hello.txt,then it shows the error.
I use the plugin: "opencode-openai-codex-auth" and the model is "GPT 5.1 Codex"

By the way, If I use the model:"GPT 5.1 Codex Max Medium (OAuth)"
the UI will broken.

All of the above errors since the version "1.0.130".
If I rollback to the version 1.0.129, everything is OK.
