This issue might be a duplicate of existing issues. Please check:

  • #7843: CLI hangs on second run (Deadlock) on Linux ARM64 - deals with second run issues
  • #9223: When installing OpenCode in Debian on Termux, the first launch succeeds but the second fails - similar pattern of first success then failure on second attempt
  • #3143: Opencode crashes on second run in Termux/Proot distro - another case of second run failures
  • #7692: JSON Parse Error with Zhipu GLM-4.7 - stream chunk concatenation issues that could cause model file parsing errors

Since you mention using transformers and that it worked for weeks then suddenly started having issues on the second request, also check:

  • #9870: missing dependencies in cached node_modules
  • #10115: Only the last plugin loads - if using oh-my-opencode, multiple plugin issues
  • #10355: opencode.json plugins version overwritten on startup

Feel free to ignore if none of these address your specific case.

Found the issue. The problem was the plug open code-agent-skills. By removing that plugin and restarting my opencode session, the issue was resolved.
