Description

When a new custom provider is created there is no icon in the Opencode Desktop UI.

PR #12517 solves with an Generic Icon

Steps to reproduce

  1. Create a custom provider and custom model in the opencode.json config
  2. Open Opencode Desktop or run opencode web
  3. Select the custom model
