fix(mcp): capture server initialize instructions and record them for the model

MCP's `initialize` result carries an optional `instructions` string, the
spec's channel for server usage guidance. Both Codewhale clients dropped
it. Plugins that ship an MCP server can now supply their guide through
this standard field; there is no plugin-specific channel.

Capture (both clients):
- codewhale-mcp: `sanitize_server_instructions` keeps a string only
  (non-strings are ignored with a debug log, handshake still succeeds),
  drops control and bidi-override chars except newline/tab, trims, and
  caps at 4 KiB on a char boundary with a "[truncated]" marker.
  ChildProcessMcpClient stores it per connection;
  `McpManagedClient::server_instructions` and
  `McpManager::server_instructions` expose it. A stopped server's
  client, and its guidance, go away together.
- codewhale-tui: McpConnection::initialize stores the same sanitized
  value (the tui now depends on codewhale-mcp directly for the shared
  sanitizer; it was already a transitive dependency via codewhale-core).
  A reconnect builds a new connection, which clears the old guidance.
  `McpPool::model_server_instructions(visible)` returns guidance only
  for ready, allowed, still-authorized servers that own at least one
  enabled tool the caller says is visible.

Placement decision: recorded in session history, not a system-prompt
section. Codewhale pins a byte-stable system prompt
(compose_stable_system_prompt is synchronous and has no MCP state), and
volatile workspace state already goes into logged user history after
the frozen prefix. The workspace-trust note is the precedent. MCP boot
is lazy (#6033), so servers connect mid-session. Rebuilding the pinned
prompt from pool state would re-pin the prefix every time a server
connected, and it would pull an async pool into the sync prompt
composer. Instead, at the existing pre-request seam in the turn loop
(right after refresh_boot_mcp_catalog), Engine appends one runtime
event `<codewhale:runtime_event kind="mcp_server_instructions">` via
add_session_message. That keeps one prompt authority and one turn loop.
- Eligibility uses the turn's final model-facing catalog, which the
  allow/deny posture has already narrowed. A server whose tools are all
  denied or hidden contributes nothing.
- The event is labelled per server (<mcp_server_instructions
  server="...">) and preceded by a preamble. The preamble says the text
  is third-party with no authority, and that system, developer and user
  instructions take precedence. Server text cannot close or forge the
  envelope.
- It is cache-stable. A new event is appended only when the rendered
  guidance differs from the latest one in history. If the last server
  goes away, a single withdrawal notice is appended. If compaction drops
  the event, the next request re-records it.
- It is auditable. The event is persisted with the session and rendered
  in the transcript as a System cell ("MCP server guidance shown to the
  model"). A Status line names the servers when guidance is added.

Tests (focused, local):
- cargo test -p codewhale-mcp --lib -- instructions: 2 passed, 0 failed
  (sanitize/trim/cap unit test; real-child handshake with string and
  non-string instructions plus manager exposure and clearing on stop)
- cargo test -p codewhale-tui --lib --all-features -- instructions:
  36 passed, 0 failed. This includes the 4 new tests: sanitized capture
  at initialize, non-string not failing the handshake, all-denied server
  excluded, and the request messages carrying the labelled section once
  plus the withdrawal.
- cargo test -p codewhale-tui --lib --all-features -- runtime_handoff
  tui::history mcp::tests: 373 passed, 0 failed
- rustfmt --edition 2024 --check on touched files: clean
Not run: the full npm test / check:web gate, and clippy.
