Documentation fixes:
* ctors.c, format.c, section.c, reloc.c: append " -" to item tags.

* doc.str (DOCDD): Defined. Adds text to output.
(bodytext): Put bulletize before kill_bogus_lines.
* chew.c (bulletize): End itemization after a blank line, to
prevent following text from being swallowed up in an item.
