Flyinghead 0a526d7c94 gdb: fix race condition when stepping. Improve vCont handling ...

Work around race condition when stepping (CpuRunning still true in
Sh4Executor::Step() on emu thread since Step() hasn't be called yet in
main thread).
Don't advertise vCont commands that aren't supported.
Range stepping end is exclusive.
