Rix d0fc51fabc mdec: reject unknown options, add rawdecode -order, review fixes 
Every command now checks its options against the ones it knows, which
the CLI contract at the top of mdec.cc already promised. rawdecode can
read the column order rawencode -order column writes. The BS decoder
helpers in dct.cc move into the anonymous namespace, and the
ReleaseWithClangCL configuration of mdec uses ClangCL like the other
tools.
