port map: the ban entry constructor is BanEntry::new

BanListEntry.<init> was the one method of a class this batch maps that the
file never answered, so the ledger still counted it missing. Both vanilla
constructors land in BanEntry:🆕 a missing source becomes (Unknown), and
the createdAt argument is not taken but stamped with now, with a file's stored
date kept by fields() on the read path.
