jalja27andjalja27 0b88599463 fix(agents): answer Qwen Code's questions in the question card (#1990) ...

Qwen Code sends AskUserQuestion questions as `_meta.qwenQuestions` and expects `answers` keyed by question index beside its Submit option, so its questions never reached the question card and Submit answered with nothing. The card now reads those questions and replies in that shape; skipped questions cancel the request, and bypassed approvals no longer auto-submit any request that carries questions.
