Steps to reproduce

Completely without network environment, local pip source, deploy dify 1.3.1 using local plugin to install ollama: https://marketplace.dify.ai/plugins/langgenius/ollama

✔️ Expected Behavior

Add model normally

❌ Actual Behavior

The add model page prompts for successful modification, but refreshing or expanding the ollama model list still leaves it empty
