Provide a description of requested docs changes

Background

The current pull request template needed improvement to ensure contributors follow our guidelines and processes before opening PRs. We identified that contributors sometimes submit PRs without reading the contribution guidelines or having an assigned issue.

Changes Made

  1. Added a prominent warning section at the top of the template using GitHub's alert syntax [!IMPORTANT]
  2. Consolidated multiple pieces of information into a single, clear checklist:
    • Reading contribution guidelines requirement
    • Issue assignment requirement
    • Issue linking syntax guidance
