LibretroAdmin 75599abff9 runloop: back off and eventually give up when the GPU device keeps being lost
A driver that lost its GPU device set VIDEO_FLAG_GPU_DEVICE_LOST and
the runloop rebuilt the video driver at once, every time. That is the
right answer to a device lost once - a TDR, a driver update - which
comes back on the first rebuild. A device that is lost again straight
after being rebuilt is a failing driver or GPU, and rebuilding on it
every frame was a busy loop of full driver reinitialisations, each of
which failed the same way.

The first loss is still rebuilt immediately. A loss following it is
retried with a doubling delay, from one second up to sixteen, and
after six in a row the rebuild is abandoned with an on-screen message
until a minute has passed. A loss a minute or more clear of the
previous one is a fresh incident and starts the count again.

Falling back to a different video driver is left as a policy choice:
this only bounds the retry loop.
