docs/tasks/README.md pointed at docs/open-issue-handoff.md as "source of
truth for the open-issue picture", but every ticket that document tracked
(#266, #267, #268, #269, #291) is closed, and planning for this project
lives in GitHub milestones rather than in a checked-in file.
Found while verifying a docs triage pass: the destination had already been
annotated as a dated snapshot, but the pointer to it had not, which is the
worse half of the problem -- a stale signpost sends people to a document
they then trust.
AvP's first 31 frames are the 320-wide title screen, so the frame-0-only
assertion `w > 320` failed on a faithful run. yarc is not a 320-only title
either (only its frame 0 is narrow), so the `band_width == 0` expectation was
wrong too. Gate now scans every frame, checks the sentinel field set on narrow
frames, and covers the exit-2 error paths.

§9 records #278–#281 as history and moves the untested push-action risk
under #252. docs/tasks/ holds self-contained prompts for T0–T5.
