Conversions such as %zu/%zx previously raised ValueError (unsupported format character). Consume arguments per pointer size and strip the modifier from the emitted format string. Also track removed characters so splices stay index-correct when a format string contains several ll conversions. Single-character conversions (%S, %P, %p and wide %%s) now apply the same removed-character offset when rewriting the format string, so their replacements land in the right slot after an earlier ll or size-modifier splice.
