The "Multiple Azure OpenAI Chat Completions" section of the notebook
does not work as is. It returns "TypeError: tuple indices must be
integers or slices, not str". The goal of this PR is to fix this error.

### Description

<!-- Describe your changes, the overall approach, the underlying design.
These notes will help understanding how your code works. Thanks! -->

Currently, the notebook passes a list of tuples to the
.complete_chat_async method. This method actually expects List[Dict[str,
str]].
