This change adds a python connector for Azure OpenAI chat completions
with data, which was already supported in the .NET SDK.

### Description

<!-- Describe your changes, the overall approach, the underlying design.
These notes will help understanding how your code works. Thanks! -->

Add connector for Azure OpenAI chat completions with data and associated
sample code for how to use it. Some minor changes were made to the SK
function to handle the "tool" message that contains the citations and
other information returned from the chat completions extensions API.
