James Westbrook 2ce041fe25 fix: is_newer explicit returns, CHANGELOG guard, pipefail consistency 
- Replace implicit return in is_newer with explicit return 0/1 for robustness
- Add CHANGELOG.md guard to detect missing '## [' heading before awk insert
- Add set -euo pipefail to workflow steps for consistency
