Changed builtin dlls file names to make it clear they are not normal

Unix libraries, and install them in $libdir/wine instead of $libdir to
avoid name conflicts in /usr/lib.

Exported CALL32_Regs from ntdll, renamed to __wine_call_from_32_regs

to follow the naming convention for this type of functions.
Split 16 and 32-bit relay functions into separate files, and moved the
32-bit ones to dlls/ntdll.

Split request and reply structures to make backwards compatibility
easier.
Moved many console functions to dlls/kernel, added code page support,
changed a few requests to behave properly with the new protocol.
