JPEG 2000 decoder for DCinema

Based on the 2007 GSoC project from Kamil Nowosad <k.nowosad@students.mimuw.edu.pl>
Updated to current programming standards, style and many more small
fixes by Diego Biurrun <diego@biurrun.de>.
