55dc68c012 USB: storage: Reject bogus max LUN values
Some mass storage devices return a bogus value in response to a Get Max LUN
request. The Iomega Jaz USB Adapter responds with 0x10, hence my recent
patch to use the US_FL_SINGLE_LUN quirk for it.

The USB MSC Bulk Only Transport document says "The device shall return one
byte of data that contains the maximum LUN supported by the device."

Since the LUN field in the command block wrapper is only 4 bits wide, it
might be helpful to report too-large LUN values in the kernel log, and
assume max LUN is actually 0. That could get some devices which currently
need the US_FL_SINGLE_LUN quirk to work.

5e4ccd9ee2 usb: ehci/ohci-platform: use SIMPLE_DEV_PM_OPS to support hibernation
ehci/ohci-platform just define .suspend/.resume functions for dev_pm_ops,
but in order to support both STR(suspend-to-ram) and hibernation, other
callbacks such as .freeze/.thaw are also required.

Registering all required callbacks for both STR and hibernation can
be done by SIMPLE_DEV_PM_OPS macro function.

a4760b3413 USB: OHCI: Eliminate platform-specific test in ohci.h
The bcm63xx and bcm3384 platforms need to set OHCI_QUIRK_FRAME_NO, but
they are non-PPC platforms and don't enable CONFIG_PPC_MPC52xx.
Therefore this patch changes the code that uses OHCI_QUIRK_FRAME_NO,
making it not depend on CONFIG_PPC_MPC52xx.

Also, rephrase the comments describing OHCI_QUIRK_FRAME_NO and the
related PSW endian swap.

11a7e59405 usb: ehci: add ehci_port_power interface
The current EHCI implementation is prepared to toggle the
PORT_POWER bit to enable or disable a USB-Port. In some
cases this port power can not be just toggled by the PORT_POWER
bit, and the gpio-regulator is needed to be toggled too.

This patch defines a port power control interface ehci_port_power for
ehci core use, it toggles PORT_POWER bit as well as calls platform
defined .port_power if it is defined.

e28e2f2f7c uas: Make uas work with blk-mq
With uas over usb-3 the tags inside the uas iu-s must match the usb-3 stream
ids, and those go from 1 - qdepth.

Before blk-mq calling scsi_activate_tcq(sdev, qdepth) guaranteed that we would
only get cmnd->request->tag from 0 - (qdepth - 1), and we used those as
uas-tags / stream-ids.

With blk-mq however we are guaranteed to never get more then qdepth commands
queued at the same time, but the cmnd->request->tag values may be much larger,
which breaks uas.

This commit fixes this by generating uas tags in the 1 - qdepth range ourselves
instead of using cmnd->request->tag.

While touching all involved code anyways also rename the uas_cmd_info stream
field to uas_tag, because when using uas over usb-2 streams are not used.

e50a322e51 usb: Do not re-read descriptors for wired devices in usb_authorize_device()
This patch modifies the usb_authorize_device() function such as that it does
not reload the device descriptor for wired devices. The reasons for this
are as follows:

* Some devices dislike the master requesting the descriptor from them twice,
  failing on the usb_get_device_descriptor() call with -ETIMEOUT. Observed this
  on my Pretec 16GB flash drive (4146:ba65).

* Malicious device could send two different descriptors - one before
  authorization, used by userspace to determine whether to authorize it and
  second to be actually used by the kernel when determining which drivers to
  bind.

6d40500ac9 usb: ehci/ohci-exynos: Fix of_node_put() for child when getting PHYs
On enabling CONFIG_OF_SELFTEST which enables CONFIG_OF_DYNAMIC,
we found out that while getting PHYs for the controller we were
doing an extra of_node_put on the child node in our routines -
exynos_e/ohci_get_phy().
This child is however already put by of_get_next_available_child()
which does a of_node_put() on the "prev" node. So there's no point
in putting the same node again in our routine.

This is a set of six patches consisting of two MAINTAINER updates, two scsi-mq
 fixs for the old parallel interface (not every request is tagged and we need
 to set the right flags to populate the SPI tag message) and a fix for a memory
 leak in scatterlist traversal caused by a preallocation update in 3.17) and an
 ipv6 fix for cxgbi.
