chore(release): fix 0.1.2 release blockers

- package.json: restore scoped name @heretek-ai/opencode-rea (regression from merge 974c8dd)
- test/unit/docs.test.ts: pin expected version to 0.1.2
- src/index.ts: re-export real type names (RuntimeEffect, ReaOptions, RuntimeOption, ParsedSkillFile)
- test/unit/readiness.test.ts: add missing add() to ToolEditorLike fake

Verified: tsc check clean, build clean, unit 91/91, integration 19/19, e2e 8/8
