0e1d9dbd8d + Flush HTML method dump file after writing it. Otherwise, the file is 
truncated if EF dies while compiling.

+ Print a placeholder disassembled instruction rather than assert-botching,
  so that the -html option still works.

1ed17c58cd initial steps toward moving cvs pull information out of nglayout.mak. 
Also, change pull_all to pull using module information instead of
each directory individually.
