At first glance, it might look like this would change behavior, since
FinishReflowChild passes aReflowInput to DidReflow, which in turn
notifies aReflowInput's mPercentBSizeObserver. However, if you examine
how the mPercentBSizeObserver is propagated, it can only be set for the
anonymous block wrapping the children of table cells, the children of
table cells, or additionally a child of a table wrapper frame that has
it set (i.e., a table or its caption, when logically a child of a table
cell). Since all of the frames for which this is being changed are
either internal table elements that are inside of the table, or are
things that can never be a descendant of a table at all, there should be
no change in behavior.

It seems better to convert this before adding a new flag (in bug
1547759) and risking replacing the wrong 0 with a flag.

Per the discussion in:
https://groups.google.com/d/msg/mozilla.dev.platform/P79pwa9z5m8/iPYPAWPHCAAJ
They should be CamelCase, and that's what most of them already do. This converts
the rest, which are a few.
For the ones that already used `e` or `k` prefixes, I've mostly done:
for file in $(rg Type::e layout | cut -d : -f 1 | sort | uniq); do sed -i 's#Type::e#Type::#g' $file; done
For the ones that used uppercase, I've removed the prefix if it was already in
the type name, and turn them into CamelCase.

Additionally, this patch makes `nsFrame.h` stop including `nsIPresShell.h`
and makes each users include `mozilla/PresShell.h` instead. So, this improves
rebuild performance of `nsIPresShell.h` (and `mozilla/PresShell.h` in the
future).
Note that due to `nsIFrame::PresShell()`, `mozilla::` prefix is necessary for
`PresShell` in a lot of classes which are derived from `nsIFrame` even in
`.cpp` files.

Except retrieving from weak reference, `nsIFrame` should treat
`mozilla::PresShell` directly rather than via `nsIPresShell`.

This is more consistent with what the Rust bits of the style system do, and
removes a pointer from ComputedStyle which is always nice.
This also aligns the Rust bits with the C++ bits re. not treating xul pseudos as
anonymous boxes. See the comment in nsTreeStyleCache.cpp regarding those.
Can't wait for XUL trees to die.

Both of reftests in this commit are based on an exmaple [1] in the Viewports
Explainer written by David Bokan.
position-fixed-out-of-view.html fails without the fix because the position:fixed
element is rendered at the right edge of the visual viewport so that it's
visible in the first place.
position-fixed-on-minimum-scale-size.html does NOT fail without the fix either
because the position:fixed element sticks at the right edge of the visual
viewport so that it still be there even after the visual viewport offset has
been changed.

Summary: Really sorry for the size of the patch. It's mostly automatic
s/nsIDocument/Document/ but I had to fix up in a bunch of places manually to
add the right namespacing and such.
Overall it's not a very interesting patch I think.
nsDocument.cpp turns into Document.cpp, nsIDocument.h into Document.h and
nsIDocumentInlines.h into DocumentInlines.h.
I also changed a bunch of nsCOMPtr usage to RefPtr, but not all of it.
While fixing up some of the bits I also removed some unneeded OwnerDoc() null
checks and such, but I didn't do anything riskier than that.
Since fixed position elements are now scrollable, we need to ensure that
they're drawn into the viewport that they're attached to.

The underlying issue here is that the rule that makes the custom content
container abspos is in ua.css, and we don't load that for SVG documents.
Similarly, this fixes all the markers and such, that would be inline because of
the lack of html.css etc.
This fixes the root cause of bug 1462742, as well, so I added a few assertions
that should replace the wallpaper there. The reason we're guaranteed to be oof
is because top-layer implies that via StyleAdjuster::adjust_for_top_layer.
As an aside, the fact that AccessibleCaret calls into InsertAnonymousContent
from frame construction makes me extremely nervous, but it already does all sort
of other pretty nasty stuff... I'll file and fix.
