refactor(goal): delete the first-generation Stop-hook Goal implementation (#11900)

* refactor(cli): pause the Goal runtime directly when the Stop-hook cap fires

The ACP session still reached into the first-generation Goal hook for two
things, and both had stopped doing anything:

- `getStopHookContinuationReason` only differed from the hook output's own
  `getEffectiveReason()` when the output carried the legacy hook's id key,
  and nothing registers that hook any more. Core's client already reads
  `getEffectiveReason()` for the same decision.
- `abortGoalForStopHookCap` cleared a Goal from the in-memory legacy store,
  which has no writer, so it always returned false and the session always
  fell through to pausing the canonical runtime.

Call `getEffectiveReason()` and pause the runtime unconditionally. No
behaviour changes; this removes the last production import of the legacy
Goal hook so the files themselves can go.

* refactor(core): delete goalHook, goalJudge and activeGoalStore

These three files were the first Goal implementation: a blocking Stop hook
that asked a transcript judge whether the condition held, backed by an
in-memory store of the active goal. The v3 runtime replaced them on every
surface, and the previous two slices cut the last callers, so nothing
registers the hook, nothing writes the store, and the judge never runs.

Delete them with their tests and the loop integration test, drop their
re-exports from the goals barrel, and remove them from the legacy filename
allowlist. The legacy `ActiveGoal` wire type already lives in
goal-legacy-projection.ts and is untouched, as are the compatibility
projections built on it.

* docs(goal): drop references to the deleted Stop-hook files

Comments, a test name, a design note and a verification inventory still
named goalHook.ts, goalJudge.ts or MAX_GOAL_ITERATIONS. Point them at what
remains, or drop the lines that only listed code that no longer exists.

* test(cli): stop pinning the legacy Goal hook's joined Stop reason

The case built a Stop hook output carrying `qwenGoalHookId`, the key only
the deleted Goal hook callback ever wrote, and asserted that its stop
reason and reason were joined. No hook can produce that output any more.

Pin what a real blocking Stop hook gets instead: when it gives both a stop
reason and a reason, the continuation carries the stop reason, which is
what `getEffectiveReason()` returns.

## Problem
`/goal <objective>` accepts any non-empty string. The objective is judged by an independent verifier that only sees transcript evidence (`goalJudge.ts`, `goal-verifier.ts`): printed text cannot prove that tests passed or files changed, claims about user actions need a real user message, and an objective nobody can evidence keeps the loop running until a limit stops it. Nothing in the product tells users this. The only guidance was the web-shell placeholder `all tests pass and the lint is clean`, which itself has no check attached.

Read the conversation transcript above carefully, then judge whether the
user-provided condition is satisfied.
Your response MUST be a JSON object with one of these shapes:
- {"ok": true, "reason": "<why the condition is satisfied>", "evidence": ["<exact assistant-output or tool-result excerpt>"]}
- {"ok": false, "reason": "<quote what is missing or what blocks the condition>"}
- {"ok": false, "impossible": true, "reason": "<explain why the condition can never be satisfied>", "evidence": ["<exact assistant-output or tool-result excerpt>"]}
Always include a "reason" field. A terminal verdict (satisfied or impossible)
MUST also quote evidence with one or more short, exact, verbatim excerpts from
visible assistant output or tool results in the "evidence" array. User prompts,
the goal condition, and hidden reasoning are not evidence. If the transcript
does not contain clear evidence that the condition is satisfied, return
{"ok": false, "reason": "insufficient evidence in transcript"}.
Only use {"ok": false, "impossible": true} when the condition is genuinely
unachievable in this session: for example, it is self-contradictory, depends on
an unavailable resource or capability, or the assistant has exhausted reasonable
approaches and the transcript confirms there is no path forward. The assistant
claiming the goal is impossible is evidence, not proof; independently confirm
the condition is genuinely unachievable rather than deferring to the assistant's
self-assessment. Do not use it just because progress is slow or evidence is
currently missing. When in doubt, return {"ok": false} without "impossible".`;

* Integration test for the /goal Stop hook loop.
*
* This intentionally does NOT boot `LlmClient` or the full hook runner.
* It exercises the seam that matters for the spec criterion:
*
* "after `/goal <condition>`, a normal attempt to stop must be intercepted
* by the same Stop hook loop used by configured hooks, and the goal must
* auto-clear only when the judge says the condition is satisfied."
*
* Concretely we verify:
* - `registerGoalHook` wires a Function hook into the session's hook system
* under the `Stop` event with a wildcard matcher (so it matches any Stop).
* - When the hook callback runs and the judge says "not met", the response
* shape is `{decision:'block', reason:<controlled prompt>}` — which
* `client.ts`'s `isBlockingDecision() || shouldStopExecution()` interprets
* as a continuation request. The judge's free-form diagnostic stays in
* active-goal state and must not become the next model instruction.
* - When the judge says "met" on a later iteration, the hook returns
* `{continue:true}`, clears the store, and notifies the terminal observer
* with stats (iterations, durationMs) — exactly what the UI needs.
* - The hook is removed from the session manager once the goal is achieved
* so a subsequent Stop event would not re-trigger the judge.
