[X86] Cleanup ShuffleDecode implementations. NFCI.

- Remove unnecessary includes from the headers
 - Fix cppcheck definition/declaration arg mismatch warnings
 - Tidyup old comments (MVT usage was removed a long time ago)
 - Use SmallVector::append for repeated mask entries
