Reid Kleckner 5b1c9f3223 Remove LLVM_NOEXCEPT and replace it with noexcept 
Now that we have dropped MSVC 2013, all supported compilers support
noexcept and we can drop this portability macro.

Peter Collingbourne 84e27c2068 Add FIXMEs to all derived classes of std::error_category. 
This helps make clear that we're moving away from std::error_code.

David Majnemer c976146d31 llvm-cxxdump: Rename llvm-vtabledump to llvm-cxxdump 
llvm-vtabledump has grown enough functionality not related to vtables
that it deserves a name which is more descriptive.
