* fix(database): keep simulated decisions from suppressing real bans

Include simulation state when comparing overlapping decisions so a longer
simulation cannot suppress a real ban or its deletion from the stream.

Cover startup and delta delivery, expiry, and overlapping real bans with
database and HTTP regression tests.

* fix(database): deduplicate simulated decisions when requested

Keep real decisions isolated by default and include simulated decisions in
deduplication when requested. Preserve the simulation filter across stream
queries and pages.

Use NOT EXISTS with a covering index to limit the query cost, migrate both
legacy indexes, and flatten the regression test tables.

Tested with make test (TEST_LOCAL_ONLY=1), make lint, Linux and Windows
builds, and database and migration checks on SQLite, MySQL, PostgreSQL
and MariaDB.
