Mike Ryan d330f814f8 host: use average RSSI when calculating signal in LE
The previous code used determine_signal_and_noise(), which assumes that
packets are continuously streaming in (i.e., rx mode for BR). This
solution doesn't work for LE since packets are bursty. Additionally we
currently have no way of capturing noise values in LE.

We simply convert the max RSSI measured by Ubertooth and set the noise
value to INT8_MIN. It's a little gross but the best we can do.

Mike Ryan 1ee8434006 usb: rssi types are int8_t instead of char
On some ARM platforms, including Raspberry Pi, char is treated as
unsigned. This was causing RSSI measurements to be incorrectly reported
as very large values rather than the correct negative values.

Mike Ryan 96bc816a0e host: change char to int for small signed integers
Using a char instead of an int is at best dumb, and at worst can lead to
unexpected behavior on platforms where char is unsigned char. See next
commit for an example of that.

Mike Ryan 3612176b93 le: disable USB interrupts while touching USB data structures
If a USB interrupt fires while we're modifing a USB data structure (such
as enqueuing a packet in the FIFO), the interrupt handler can modify the
data structure we're touching. This is akin to a multithreaded app not
locking shared data structures. We avoid this by hackily disabling USB
interrupts for the duration that we're modifying the data structure.

For LE, this works because the host code polls Ubertooth in a tight
loop. If the poll fails, the host code silently ignores it and retries
again. This code should be rearchitected to handle USB without
interrupts in a safe period between packets.

Fixes #249
